OW I ? 2013 N-2013-037 <br />ms`s pp,Cs(1) <br />T`kay) L,ara Memorandum of Understanding (MOU) <br />Between Monarch Health Care and <br />the City of Santa Ana <br />1. PARTIES: The parties to this Memorandum of Understanding ("MOU") are the <br />Monarch Healthcare (hereinafter referred to as Monarch) and the City of Santa Ana <br />by and through the Santa Ana Senior, Center ("City"). <br />2. PURPOSE: The City, in conjunction with Monarch, endeavors to provide health <br />care and community services to.senior. citizens in Santa Ana, at the Senior Center <br />located at 424 W. 3rd Street Santa Ana; California 92701. <br />3. RESPONSIBILITIES: <br />A) The City of Santa Ana shall perform the following: <br />1) Reserve the back page for Monarch in each of the six (6) editions of the Santa <br />Ana Senior Center newsletter. <br />2) Provide wall space for Monarch health education display. <br />3) Distribute placemats at congregate meals or other group activities. <br />4) Recognize Monarch as a major sponsor at four (4) events at no additional <br />charge. Recognition will be in the newsletter and at an announcement at the <br />event. Santa Ana Senior Center will offer Monarch the option to be present <br />and involved in the event. <br />*May 3: Cinco De Mayo & Salsa Making Contest <br />*August 23: International Day Celebration <br />*September 13: Latino Heritage Celebration <br />*November 22: Thanksgiving & Volunteer Appreciation Day <br />5) Allow a monthly resource table, staffed by Monarch personnel. <br />B) Monarch will offer financial and educational support in 2013 as follows: <br />1) Six health tips for the senior center newsletter. Monarch sends in an <br />electronic format that is easily added to the newsletter: Publisher, 8 '/2 x 11. <br />2) Monarch shall maintain the Health Display at the Senior Center with current <br />and timely information. They will check it monthly, keep contents in order, <br />and periodically replace inserts with new information. <br />3) Deliver 1,000 health-related placemats per quarter to the Senior Center. <br />4) Monarch will participate in four (4) Senior Center events. So as not to <br />interfere with activities, Monarch and Santa Ana Senior Center will discuss <br />how they will coordinate and participate in advance of each event.
