PLEASE READ IT <br />CAREFULLY. <br />ENDORSEMENT# <br />This endorsement, effective 12:01 A.M. 06/30/2013 forms a part of Policy <br />No. GL 509 -46 -99 issued to CELLCO PARTNERSHIP DBA VERIZON WIRELESS, INC. <br />By NATIONAL UNION FIRE INSURANCE COMPANY OF PITTSBURGH, PA <br />(The above "attaching clause "need be completed only when this endorsement is issued subsequent to the preparation of tile policy). <br />NOTICE OF CANCELLATION, NONRENEWAL AND CHANGES TO CERTIFICATE <br />HOLDER <br />This endorsement modifies insurance provided under the following: <br />COMMERCIAL GENERAL LIABILITY COVERAGE FORM <br />SECTION IV — COMMERCIAL GENERAL LIABILITY CONDITIONS, 10. <br />CHANGES is amended to add the following: <br />We will mail or deliver a prior written notice of a "material change," <br />consented to by the first Named Insured shown in the Declarations and us, <br />to the first Named Insured's last known mailing address and to those entities <br />set forth in the schedule below. We will provide written notice of a <br />"material change" at least NINETY (90) days prior to the effective date of <br />"material change." <br />For purposes of this paragraph, "material change" means any of the <br />following: (i) a reduction in the Limits of Liability set forth in the policy <br />Declarations; (ii) a deletion of or addition to the list of Coverages set forth in <br />Section A of this policy; or (iii) a change in the Policy Period set forth in the <br />policy Declarations. <br />SECTION IV — COMMERCIAL GENERAL LIABILITY CONDITIONS, 12. <br />CANCELLATION is amended to add: <br />We may cancel this policy by mailing or delivering to those entities set forth <br />in the schedule below, written notice of cancellation at least: <br />,AppROVEO AS TO FO TUVt <br />LISA E.
