Exhibit A <br />SCOPE OF SERVICES — Kay Cotter <br />Provider shall conduct Creative Sculpting w/Clav & Adult ceramies-Hand building for children 9 M old - 99yrs old <br />A. Creative Sculpting with Clay class will consist of monthly session, held 1 day per week, I %2 hour per day- $45 <br />per session. A separate, one time per session material fee of $15 shall be paid directly to Provider. <br />B. Adult Ceramics/Hand building class will consist monthly session, 2 days per week, and 2 hours per day -$115 <br />per session. A separate, one time per session material fee of $25 shall be paid directly to Provider. <br />C. Classes will be throughout the year. <br />D. Provider and City staff shall mutually agree upon a schedule for classes, including the location, specific days and <br />hours when class will be held and holidays to be observed. <br />E. Provider shall provide materials, supplies, equipment, records and personnel. Provider shall be responsible for <br />cleanup of the facilities and materials and shall ensure the safety and effectiveness of instruction. <br />E. If Provider allow others to tech his/her class, those teachers must be over 21, have obtained and maintain an <br />instructor rating, and be covered by Providers insurance. Provider shall provide City with documentation to verify <br />instructor and insurance requirements <br />CLASS SIZE <br />1) Each Creative Sculpting w/Clav class must have a minimum of 7 paid students and no more than a maximum <br />of 15. <br />2) Each Adult Ceramics/1-land building class must have a minimum of 7 paid students and no more than a <br />maximum of 15. <br />3) If the minimum registration has not been reached by the second class, the class may be cancelled by mutual <br />agreement of Provider and City, without any compensation owed to Provider. <br />4) Provider will provide and be responsible for equipment, records, and personnel and clean up of the facilities. <br />CLASS FEES <br />1) Each participant shall pay $45/monthly sessions for each participant, and $15 monthly material fee for each <br />class payable to instructor. (Creative Sculpting w/Clay) <br />2) Each participant shall pay $115/monthly sessions for each participant, and $25 monthly material fee for each <br />class payable to Provider. (Adult Ceramics/Hand building) <br />3) The City shall collect registration from each participant during the registration period. Provider shall collect <br />material fees, and shall refer all interested participants to City for registration. <br />4) Provider will receive ninety percent (90`.'/0) of the total fees collected. City and Provider agree that City shall <br />retain ten percent (10%) of the fees collected as an administration fee. Compensation hereunder shall not exceed <br />$25,000.00 annually. <br />5) Provider agrees that City is entitled to audit Provider's records and classes to insure compliance with this <br />Agreement. <br />6) City shall prepare class rosters and provide a copy to Provider. Only registered participants may participate in <br />class. <br />7) Provider may not waive class participation/registration fees <br />
