City of Santa Ana <br />Scope of Work <br />Name of Organization Human Options <br />Name of Funded Program Human Options Emergency Shelter <br />Annual Accomplishment Goal <br />I. Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded program, named above, during the 12 -month contract period. <br />250 Persons <br />II. Number of unduplicated Santa Ana residents expected to be served by the funded program during the 12 -month <br />contract period. <br />18 Persons <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />ESG funds will be used specifically to provide: <br />1. Case Management services to Santa Ana shelter clients <br />2. Funds will be used to cover utilities at the shelter <br />3. Funds will be used to maintain and repair the shelter <br />4. Funds will be used to purchase food for Santa Ana shelter clients <br />Human Options Emergency Shelter provides safety and services to victims of domestic violence. The shelter <br />offers a wide range of on -site essential services which address health, drug abuse, domestic violence, <br />education, counseling, and housing. A Case Manager is assigned to each woman who enters the shelter. The <br />Case Manager will meet wt hthe individual 1 -2x /week and will assist her in obtaining transitional or <br />permanent housing; medical services; substance abuse treatment and counseling; financial assistance, child <br />case, and transporation assistance. Each woman is assigned a counselor who provides psychological <br />counseling to help her with her emotional needs and assists her in recovering from the trauma of domestic <br />violence. The counselor also provides family counseling 1x /per week to woman and their children. All women <br />participate in psycho - educational , process oriented, and life skills groups. These groups includ <br />psychoeducation about domestic violence, parenting, safety planning ,building healthy relationships, drug and <br />alcohol abuse, and financial literacy. Human Options ER Shelter additionally provides legal support. Our legal <br />advocates assist each client according to her identified needs. A legal advocate can assist in applying for a <br />restraining order, accompaniementto court, and providing legal linkages and referrals for divorce and custody <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12 -month <br />contract period per quarter.
