EXHIBIT B <br />If possible, include provisions under the Act, regulations, grant or other <br />agreements under the Act are believed to have been violated. <br />Include the remedy sought by the complainant. <br />The absence of any of the requested information shall not be a basis for <br />dismissing the complaint. <br />A copy of the complaint must be sent to the respondent and both parties notified <br />of the opportunity for an informal resolution. At each step of the complaint <br />process, the complainant must be notified in writing of the next procedural step. <br />If the complaint is resolved prior to the LWIA's formal fact- finding conference, the <br />LWIA will notify the complainant in writing of the final resolution and the reasons <br />for the action. The complainant will provide a written withdrawal of the <br />complainant 10 days of the receipt of the notice of resolution, <br />Complaints shall be filed with: <br />Anita Queen, Equal Opportunity Officer, <br />20 Civic Center Plaza, City Hall -5th Floor <br />Santa Ana, CA 92701 (7 114) 647 -5157, TDDITTY (714) 656 -2669. <br />A fact finding hearing will be conducted within 30 days of receipt of formal written <br />complaint. <br />D. Notice of Hearing <br />Upon receipt of the request for hearing, the complainant and the respondent will <br />be notified in writing of the hearing ten (10) calendar days prior to the date of the <br />hearing. The ten -day notice may be shortened with the written consent of the <br />parties. <br />The hearing notice shall be in writing and contain the following information: <br />The date of notice, name of the complainant, and the name of the party against <br />whom the complaint is filed. <br />The date, time, and place of hearing before an impartial hearing officer. <br />A statement(s) of the alleged violation(s) <br />Advise as to where information or assistance may be obtained, and the name, <br />address, and telephone number of the Santa Ana LWIA Equal Opportunity (EO) <br />Officer who can answer inquiries. <br />M <br />
