Exhibit A <br />SCOPE OF SERVICES — Geraldine Valencia <br />Provider shall provide the following classes for children and adults. <br />A. G-Stars Parent "N" Me Gymnastics and Dance class will consist of a monthly session, held 1 day per <br />week, 50 minutes per day. <br />B. G-Stars Beginning Gymnastics class will consist of a monthly session, held 1 day per week, 50 minutes <br />per day. <br />C. G-Stars Tumbling for "TEENS" class will consist of a monthly session, held 1 day per week, 50 minutes <br />per day. <br />D. Classes will be throughout the year. <br />E. Provider slid City staff shall mutually agree upon a schedule for classes, including the location, specific <br />days and hours when class will be held and holidays to be observed. <br />F. Provider shall provide materials, supplies, equipment, records and personnel, Provider shall be responsible <br />for clean-up of the facilities and materials and shall ensure the safety and effectiveness of instruction. <br />CLASS SIZE <br />1. Each Gymnastics class must have a minimurn of 5 paid students and no more than a maximum of 20/40 <br />students, or as established by mutual agrecr sent of the City and Provider. <br />2. No registration will be accepted after the second meeting of class. <br />3. In the event the minimum number of enrollees is not realized by the second meeting of the class, the class <br />shall be cancelled. Provider will be under no obligation to provide services for the cancelled class, and the <br />City will have no further obligation to pay Provider compensation for the remaining classes that were <br />cancelled in that session. <br />CLASS FEES <br />1) Each participant shall pay class registration fees as established by the City. <br />2) Provider may not waive class participation/registration fees. <br />3) Only registered participants may participate in the class, <br />4) Any materials fee shall be established by mutual agreement of the City and Provider and shall be payable <br />directly to Provider. <br />5) Any refunds to participants will be made in accordance with City policy. <br />7 <br />
