Exhibit A SCOPE OF SERVICES — Geraldine Valencia Provider shall provide the following classes for children and adults. A. G-Stars Parent "N" Me Gymnastics and Dance class will consist of a monthly session, held 1 day per week, 50 minutes per day. B. G-Stars Beginning Gymnastics class will consist of a monthly session, held 1 day per week, 50 minutes per day. C. G-Stars Tumbling for "TEENS" class will consist of a monthly session, held 1 day per week, 50 minutes per day. D. Classes will be throughout the year. E. Provider slid City staff shall mutually agree upon a schedule for classes, including the location, specific days and hours when class will be held and holidays to be observed. F. Provider shall provide materials, supplies, equipment, records and personnel, Provider shall be responsible for clean-up of the facilities and materials and shall ensure the safety and effectiveness of instruction. CLASS SIZE 1. Each Gymnastics class must have a minimurn of 5 paid students and no more than a maximum of 20/40 students, or as established by mutual agrecr sent of the City and Provider. 2. No registration will be accepted after the second meeting of class. 3. In the event the minimum number of enrollees is not realized by the second meeting of the class, the class shall be cancelled. Provider will be under no obligation to provide services for the cancelled class, and the City will have no further obligation to pay Provider compensation for the remaining classes that were cancelled in that session. CLASS FEES 1) Each participant shall pay class registration fees as established by the City. 2) Provider may not waive class participation/registration fees. 3) Only registered participants may participate in the class, 4) Any materials fee shall be established by mutual agreement of the City and Provider and shall be payable directly to Provider. 5) Any refunds to participants will be made in accordance with City policy. 7
