1.0 GENERAL <br />1.1 INTRODUCTION <br />1.2 PERIOD OF CONTRACT <br />Unless earlier terminated as allowed for in the agreement, contract term shall be for a <br />period of 12 months with 2 one-year options. The contract term is anticipated to <br />commence after City Council award of this contract and upon receipt and approval of all <br />required bonds and insurance documents. The projected contract award date is February <br />3, 2015 and may be adjusted as necessary. <br />13 RFP PROCESS AND GENERAL TERMS <br />1.3.1 PROPOSAL SUBMISSIONS -Proposals must be submitted to the Budget and <br />Accounting Section in a sealed envelope showing on the outside, the name of the <br />bidder, RFP NO, 14-077 and the proposal due date and time. All proposals must <br />be signed by an authorized representative of the vendor. <br />1.3.2 PROPOSAL WITHDRAWAL -Proposals may be withdrawn by written request <br />received before the hour set for the opening. After that time, the vendor may not <br />withdraw proposals for a period of ninety (90) days from the date of the opening. <br />1.3.3 LATE PROPOSALS -It is the responsibility of the vendor to see that any <br />proposal submitted shall have sufficient time to be received by the Budget and <br />Accounting Section prior to the proposal due date and time. Santa Ana Parks, <br />Recreation and Community Services Agency shall not be responsible for <br />proposals lost or delayed in the mail. Late proposals will be returned to the <br />vendor unopened. <br />1.3.4 REQUIRED DOCUMENTS -Vendor must return the following completed, <br />documents with their proposal: <br />1.3.4.1 Detailed proposal of services in accordance with the specifications <br />detailed in Section 4.0. Proposal should take into consideration the <br />evaluation criteria included. in Section 3.0 and should not to exceed 20 <br />pages. <br />1.3.4.2 Signed proposal form (Section 6.0); <br />1.3.4.3 A letter or printout of vendor's safety related incidents from insurance <br />company, current or previous, spanning the past three (3) years; <br />1.3.4.4 Completed vendor's references sheet (Section 7.0); <br />1.3.4.5 A resume or summary detailing vendor's qualifications, <br />accomplishments, and membership in any professional and/or industry <br />organizations; <br />1.3.4.6 A representative sample of photos of vendor's past events and/or <br />vendor's equipment; <br />1.3.4,7 A list of ride equipment owned, used acrd/or operated in vendors events, <br />with description and age of all rides; <br />1.34.8 Completed Proposal Summary and Deviations from Specifications <br />(Section 8.0) <br />1.3.4.9 Any other information the vendor may choose to submit. <br />Attachments A, B, and C are for reference only, but will be required after the contract is awarded to <br />the successful vendor. <br />0 Vendor roust submit one original and eight photocopies of their proposal. <br />
