EXHIBIT A <br />I. SCOPE OF SERVICES -Denise Robles <br />A. Provider will teach Aerobics class in Beginning and Beginning Intermediate on a monthly <br />basis for ages 15 +. <br />B. Beginning class is 2 days per week, I hour per day - $20/monthly <br />C. Begimring/Intennediate class is 2 days per week, 1 hour per day - $20/monthly <br />D. Latin Zumba Fitness class is 1 day per week, lhour per day - $15/monthly <br />E. The classes will be held throughout the year as agreed between Provider and City. <br />F. Contractor will provide beginning and Advance Aerobics classes using accepted safe and <br />healthy Aerobics techniques, warm up exercises and steps as well as cool down techniques. <br />G. Provider shall work with City staff in setting a schedule for classes, including the location, <br />specific days and hours when class will be held and holidays to be observed. <br />II, Provider shall provide materials, supplies, equipment, records and personnel. Provider shall <br />be responsible for cleanup of the facilities and materials and shall ensure the safety and <br />effectiveness of instruction. <br />II. CLASS SIZE <br />A. Each class must have a minimum of 8 paid students and no more than a maximum of 25, <br />If the minimum registration has not boon reached by the second class, the class may be <br />cancelled by mutual agreement of Consultant and City, without any compensation owed to <br />consultant. <br />III. CLASS FEES <br />A. The class fee is $20.00 a month. Anticipated revenue not to exceed $25,000.00 annually. <br />B, No refunds shall be made to participants after the first week of class unless the class is <br />cancelled by the Parks, Recreation and Community Services Agency. <br />C. The City shall collect the class fees from each participant during the registration period. <br />Consultant shall not collect fees, but shall refer all interested participants to City for <br />registration. <br />D. City agrees to pay Consultant seventy percent (70%) of the total fees within thirty (30) <br />working days after completion of the class session. City and Consultant agree that City shall <br />retain thirty percent (30%) of the fees collected as an administrative fee. <br />E. Consultant agrees that City representative shall be entitled to audit Consultant's records to <br />insure compliance with this Agreement. <br />
