City of Santa Ana CDBG Scope of Work <br />Program Year 2016-17 (July 1, 2016 - June 30, 2017) <br />Name of Organization Human Options <br />Name of Funded Program Community Domestic Violence Program <br />Annual Accomplishment Goal <br />Unduplicated Participants anticipated to be served durin2 the 12 -month contract period. <br />200 TOTAL 1 1 2001 Santa Ana Participants 1 100% 1 1801 Low Income Participants 90% <br />ruw <br />Human Options Community Domestic Violence Program intends to provide intervention and prevention program <br />services to victims of domestic violence, teen dating violence and late life family violence, or those at risk for future <br />victimization in Santa Ana. <br />Services will be provided throughout the City of Santa Ana including but not limited to Santa Ana Unified School <br />District school sites and the Corbin and Minnie Street Family Resource Centers. All services will be available in <br />English and Spanish. <br />Individual and Family Counseling <br />• Individual and family counseling services will be offered using a client centered and strength based approach. <br />The therapist funded through this grant will provide trauma informed and clinically appropriate counseling <br />services to victims of domestic violence, teen dating violence, and elder abuse. Counselling services are <br />offered on an ongoing basis and are determined by the client's therapeutic goals. On average, services are <br />offered once a week from 1-24 weeks. <br />Group Counseling <br />• Safe Dates for youth — Safe Dates is a six week dating abuse prevention education curriculum intended for <br />middle school and high school youth focused on healthy relationships. <br />• Families for Safe Dates for parents— Families for Safe Dates is a six week educational curriculum focused on <br />educating parents on healthy relationships and providing them with skills and opportunities to engage their <br />children in meaningful conversations. <br />• Senior Empowerment Group — Senior Empowerment Group is a ten week educational group for individual of <br />the age of 50 and over covering topics such as family violence dynamics, safety planning, substance abuse, co- <br />dependency, healthy relationships, and legal remedies. <br />Schedule of Performance <br />Unduplicated Estimated <br />Participants Invoicing <br />Quarter 1: JUL 1 - SEP 30 J200 <br />$ 7,295.50 <br />Quarter 2: OCT 1 - DEC 31 $ 7,295.50 <br />Quarter 3: JAN 1 - MAR 31 $ 7,295.50 <br />Quarter 4: APR 1 - JUN 30 $ 7,295.50 <br />$29,182.00 <br />Exhibit A <br />Page 1 of 1 <br />
