You should read your policy and review your Declarations page for complete information on the <br />coverages you are provided. <br />This Notice provides information concerning possible impact on your insurance coverage due to directives <br />issued by the United States. Please read this Notice carefully. <br />The Office of Foreign Assets Control ( "OFAC ") of the U.S. Department of the Treasury administers and <br />enforces economic and trade sanctions based on U.S. foreign policy and national security goals against <br />targeted foreign countries and regimes, terrorists, international narcotics traffickers, those engaged in activities <br />related to the proliferation of weapons of mass destruction, and other threats to the national security, foreign <br />policy or economy of the United States. OFAC acts under Presidential national emergency powers, as well as <br />authority granted by specific legislation, to impose controls on transactions and freeze assets under U.S. <br />jurisdiction. OFAC publishes a list of individuals and companies owned or controlled by, or acting for or on <br />behalf of, targeted countries. It also lists individuals, groups, and entities, such as terrorists and narcotics <br />traffickers designated under programs that are not country- specific. Collectively, such individuals and <br />companies are called "Specially Designated Nationals and Blocked Persons" or "SDNs ". Their assets are <br />blocked and U.S. persons are generally prohibited from dealing with them. This list can be located on OFAC's <br />web site at — hftp / /www.treas.gov /ofac. <br />In accordance with OFAC regulations, if it is determined that you or any other insured, or any person or entity <br />claiming the benefits of this insurance has violated U.S. sanctions law or is an SDN, as identified by OFAC, the <br />policy is a blocked contract and all dealings with it must involve OFAC. When an insurance policy is <br />considered to be such a blocked or frozen contract, no payments nor premium refunds may be made without <br />authorization from OFAC. <br />Form IH 99 40 04 09 Page 1 of 1 <br />
