Gay Storm will <br />provide costuming and props for her show but will not <br />provide a microphone. <br />Compensation for her work will be $300.00. <br />Enclosed with this invoice is a Vendor Information Form, a <br />Taxpayer Form W-9 and a Workers' Compensation <br />Declaration. <br />------ ---------------�- Gay Storm <br />
