City of Santa Ana CDBG Scope of Work <br />Program Year 2017-18 (July 1, 2017 -June 30, 2018) <br />Name of Organization Nati's House <br />Name of Funded Program Neutral Ground <br />Annual Accomplishment Goal <br />Unduplicated Partici ants anticipated to be served duringthe 12 -month contract period. <br />82 TOTAL F 82 1 Santa Ana Participants 1 100% 82 Low Income Participants 1 100% <br />Program and Funding Description <br />Neutral Ground staff will offer three different types of evidenced -based mentoring practices. This will provide <br />different levels of engagement and intensity that will accommodate each student's unique emotional and social <br />needs. The three -different evidence based mentoring practices are as follows: <br />1. Staff will offer one-on-one mentoring after school to youth on a daily basis. <br />2. Group mentoring opportunities through circles and breakout groups will encourage students' positive behavioral <br />growth through both an adult staff facilitator as well as through peer-to-peer interactions. <br />3. Service -based mentoring will be provided through other venues, such as after school community -service projects, <br />or post -secondary education linkages. <br />Individual case management and mentoring will be student -led with an adult. This will create a dynamic that will <br />encourage students to disclose information and seek out the Neutral Ground staff member with whom they have <br />the most connection.
