a h arm <br />your indeprnd.H hrnfth.m ch im. <br />High Acuity Notifications: NaphCare's Utilization and Case Management team closely monitors <br />inmates diagnosed with chronic and complex illness. TechCare® aids staff in this process by <br />tracking the number of off-site visits by way of a watch list. Inmate acuity level is based on the <br />severity of illness and subsequent off-site treatments. When our nurse anticipates that an <br />inmate's care will require a wide range of resources, multiple off-site trips, or extended <br />hospitalizations and treatments, a high acuity notification is sent to the appropriate leadership <br />and jail personnel. NaphCare ensures that high acuity inmates are closely monitored, which <br />reduces readmission, prolonged length of stay, and repeat surgery, as well as other medical <br />expenses. <br />Pre -Procedure Instructions to Inmates: With the <br />goal of educating inmates on scheduled <br />procedures, NaphCare's clinical support staff <br />provides inmates with medical instructions and <br />information priorto these procedures. Through <br />this process, inmates receive evidence -based <br />answers to clinical questions at the point of care. <br />Informed of their procedures, inmates are <br />prepared to ask questions and engage in <br />conversations with clinical staff regarding course <br />By following our policies, we are <br />able to shorten the length of <br />stay, preserve quality of care, <br />and enhance discharge planning <br />for return to the facility. <br />of treatment. Regular communication helps NaphCare develop <br />appropriate discharge plans and maximize on-site infirmary capabilities. <br />RETROSPECTIVE REVIEW —Occurs on all ER trips and for any questions or concerns that may <br />arise regarding the quality and appropriateness of an inmate's care. As part of our quality <br />initiative, our UR nurses and Chief Medical Officer review all emergency room visits and monitor <br />the site Medical Directors appropriate use of the on-site facilities. <br />Utilization Management Reports <br />NaphCare has unlimited reporting capabilities based on the data captured in TechCare®. We analyze <br />costs, trends, and provide reports in any format you request. In addition to providing the above services, <br />we study statistics that aid you in improving your utilization of off-site care. Our sophisticated reporting <br />capabilities, combined with our strong correctional operations experience, creates highly satisfied <br />clients.
