ci%1 c?P <br />your independent heolih=,e choice. <br />Medication Review <br />NaphCare pharmacists provide a thorough clinical review of drug orders as new <br />prescriptions are electronically sent to the NaphCare Pharmacy. As a value-added <br />service for the Jail, NaphCare's pharmacists review all orders for accuracy. They verify <br />real-time prescriptions for safe dosage, allergies, specified length of time, need for drug, <br />and duplications. By identifying duplications, we minimize the number of drugs a patient <br />needs while still ensuring high quality care. <br />Pharmacv Queue &Vital Signs <br />Current vital signs, such as blood pressure and blood glucose levels, are shown <br />on the Pharmacy Queue to alert pharmacists of any abnormal vital signs which <br />may preclude use of the prescribed drug. Within this screen, it is easy for <br />pharmacists to see when scheduled or appropriate vital signs have not been <br />taken or properly recorded; they are then able to communicate with healthcare <br />staff so that corrective action can be taken and negative outcomes can be <br />prevented. <br />Clinical Pharmacist Quality Assurance Management Process <br />When inmates first enterthe Jail, a Intake Screening is completed to gather important <br />inmate physical and mental health information up front to prevent negative outcomes <br />later. During this screening, we identify any current or needed medications. Medication <br />orders are entered into the TechCare® eMAR, ensuring that all medication activities are <br />tracked. <br />Within our eMAR, all providers have the information they need to review and approve <br />orders and make more informed decisions regarding patient care. At the time that our <br />providers review and approve orders, our Pharmacy Quality Assurance Management <br />Process begins. All of the above activities, which compose our Clinical Pharmacist <br />Quality Assurance Management Process, are completed within 24 hours of an inmate <br />entering your facility. Finally, when all medications are approved and our pharmacists <br />have reviewed in detail all medication information, our nurses administer medications <br />to your inmates in compliance with NCCHC standards for care and the manner which <br />you have requested. <br />When a provider reviews and approves orders in the TechCare° eMAR, our Pharmacy <br />Quality Assurance Management Process begins.
