7) Budget outlining how remaining Federal and non -Federal funds will be <br />expended; <br />8) Plan for completion, including milestones and timeframes for achieving each <br />milestone and the position/person responsible for implementing the plan for <br />completion; and <br />9) Certification that the activity/activities will be completed within the extended <br />period of performance without any modification to the original Statement of <br />Work, as described in the investment justification and approved by <br />DHS/FEMA. <br />Recipients must submit all proposed extension requests to DHS/FEMA for review and <br />approval no later than 120 days prior to the end of the period of performance. In <br />accordance with GPD policy, extensions are typically granted for no more than a six <br />month time period. For more details please review Information Bulletin 379 at <br />http•//www,fema.goL/Rdaovernment-rrantlbulletinslinfo379pdf <br />National Preparedness <br />DHS/FEMA coordinates with local, state, territory, Tribal, and Federal governments as <br />well as the private and non-profit sectors to facilitate an all-of-nation/whole community, <br />risk driven, and capabilities -based approach to preparedness. This risk driven, <br />capabilities -based approach is grounded in the identification and assessment of risk <br />through the THIRA. For additional information on THIRA, please refer to: <br />ham -//www femc gov/threat-and-hazard-identification-and-risk-assessment. HSGP grant <br />recipients must submit a THIRA annually to ensure that the community's shared <br />understanding of risk evolves to account for changes in the risk landscape, including <br />successful mitigation efforts, emerging threats, hazards, and associated <br />consequences. Information on the National Preparedness System can be found in the <br />National Preparedness System Description (released Nov 2011), at <br />http://www.fema.govinational-orel2aredness-system. Additional details regarding the <br />National Preparedness System and how it is supported by HSGP can be found in <br />Appendix B -Program Priorities. <br />Payments <br />DHS/FEMA utilizes the Payment and Reporting Svstem (PARS) for financial reporting, <br />invoicing and tracking payments. <br />DHS/FEMA uses the Direct Deposit/Electronic Funds Transfer (DD/EFT) method of <br />payment to recipients. To enroll in the DD/EFT, the recipients must complete a Standard <br />Form I I99A, Direct Deposit Form. <br />Monitoring <br />Recipients will be monitored on an annual and as needed basis by DHS/FEMA staff, both <br />programmatically and financially, to ensure that the project goals, objectives, <br />performance requirements, timelines, milestone completion, budgets, and other related <br />program criteria are being met. <br />28 <br />FY 2015 HSGP NOFO <br />
