City of Santa Ana CDBG Scope of Work <br />Program Year 2017-18 (July 1, 2017 -June 30, 2018) <br />Name of Organization Santa Ana Unidos <br />Name of Funded Program Boxing Youth/Neighborhood Crime Prevention Initiative <br />Annual Accomplishment Goal <br />Unduplicated Participants anticipated to be served during the 12 -month contract period. <br />100 TOTAL 1 100 Santa Ana Participants 1 100% F 75 Low Income Participants 75% <br />Program and Funding Description <br />We serve 100 at -risk youth between the ages of 12 and 18 through our core intervention and prevention program <br />and approximately 200 with our outreach and education programs annually. As a youth development non-profit, we <br />use year-round, boxing and mixed martial arts sports to engage underserved students in long-term mentor based <br />relationships. <br />Our curriculum has four primary areas where we focus our evaluation efforts: Education or preparation for life path; <br />Community participation; Economic stability; and Parent empowerment. Our mentor/coaches focus each young <br />person's strengths to help them realize the potential within and with others and their community. Our program <br />activities are action oriented. <br />Service are provided in several methods: 1) One-on-one is initially provided for participants referred by School <br />Counselors, Probation, and OC Workforce Investment Board and then referred to group counseling and activities; 2) <br />All participants received education, life skills, financial literacy, relationship building and other prevent services in <br />group settings; 3) Home visits and phone counseling/referrals are conducted on an as needed basis; and Community <br />workshops/events are conducted four times a year in the community to provide community outreach, civic <br />engagement and community action activities. <br />Target population groups: Our target group are youth 12-18 years who live in Santa Ana. We take referrals from <br />community partners and youth who enroll from community outreach events. Currently, we are working with youth <br />from Jerome Community Center, Latino Health Access Community Center, EI Salvador Park Community Center, Delhi <br />Community Center and Santa Anita Community Center and surrounding neighborhoods.
