Community -Based Program Experience <br />Super Clinik has involved itself with non-profit organizations such as, collecting donations for <br />OC Food Bank and Toys for Tots. We have matched 100 percent of the donations collected and <br />continue to commit future donations to support the non-profit organizations. We believe in being <br />compassionate by giving back to our patients and has offered the following compassionate <br />programs: <br />• Quarterly Events allowing patients to obtain their medical cannabis at a <br />discounted rate of 95%-99%. <br />• Provided free medical cannabis to patients with terminally ill conditions. <br />The Operator of Super Clinik, Joanne Nguyen has volunteered with the following organizations: <br />• Operation Be Kind in Santa Ana that collects and donates needed goods to children. <br />• Volunteered with Orange County and Los Angeles Soup Kitchens. <br />• Fed the Homeless at 7 Seas Restaurant in Garden Grove, CA <br />• Volunteered with the community of Santa Ana for their Thanksgiving and Christmas <br />Events three years in a row to help the Homeless. <br />• Assisted local temples with donations for their Orphanage Program in Vietnam <br />She has also received the following certificates of recognition for her volunteering services: <br />Recognition of Volunteer Service during USNS Mercy Humanitarian Mission Certificate <br />of Special Congressional Recognition, 2007 <br />Recognition of Volunteer Service during USNS Mercy Humanitarian Mission Certificate <br />of Special Congressional Recognition, 2008 <br />Certificate of Special Congressional Recognition, 2009 <br />Recognition of Volunteer Service during the USS Peleliu Humanitarian <br />Good Neighbor Policy <br />Super Clinik has the responsibility to be the best neighbors we can be. We continue to <br />work with our neighbors and the community on issues they perceive as a problem and <br />address them in a timely manner. The building and surrounding areas are monitored <br />at all times to prevent loitering and any other potential disturbances to the quiet <br />enj oyment of the neighbors and surrounding businesses. We anticipate, given our <br />commitment to being model community members and our world-class operating <br />standards, our establishment contributes positively to the safety and security of the <br />area surrounding our facility. In keeping with our commitment to community, we <br />implemented "good neighbor" trainings for our staff emphasizing sensitivity to the <br />concerns of neighbors, and require our staff and patients to adhere to a code of <br />neighborhood conduct. <br />5 <br />
