ARTICLE XI <br />11.0 EMPLOYEE INSURANCE <br />11.1 Health Insurance. Effective January 1, 2018, the City shall contribute the following to <br />insurance coverage: <br />Employee Only <br />Employee + 1 <br />Employee + 2 or more <br />11.2 Dental Insurance. <br />$654.00 per month <br />$1,307.00 per month <br />$1,700.00 per month <br />A. The City shall continue to contribute the current maximum amount of $90 per <br />month per SAMA employee for employee dental insurance premiums. Effective <br />January 1, 2016, the City contribution amount will be increased to a maximum of <br />one hundred ten dollars ($110) per month per employee. <br />B. Any contribution necessary to maintain benefits under said dental plans in excess <br />of the amount set forth above shall be borne entirely by the employee. <br />11.3 Long -Term Disability (L.T.D.) hnsurance. The City shall continue to pay one hundred <br />percent (100%) of the premium cost for a 60 -day elimination period long term disability <br />insurance plan for employee covered by this MOU. <br />11.4 Life Insurance. The City shall continue to pay one hundred percent (100%) of the <br />premium cost for term life insurance coverage under the policy it maintains on behalf of <br />employees in order to provide employees covered by this MOU with life insurance <br />coverage in an amount equal to twice such employee's annual rate of salary to a <br />maximum of three hundred thousand ($300,000) provided said employees can provide <br />evidence of insurability of coverage, if so required by the teens and conditions of said <br />tern life insurance policy. <br />In the event any such employee is determined to be ineligible for said insurance <br />coverage, the City will attempt to provide as much coverage as may be obtained at <br />reasonable cost not to exceed the actual monthly premium amount allotted for said <br />coverage without having to provide evidence of insurability. <br />37 <br />
