Recitals: A. City and Consultant entered into Consultant Agreement N2001 -193, dated October 23, 2001, (hereinafter "said Agreement ") by which Consultant has provided counseling services to inmates housed at the Santa Ana City Jail. B. Said Agreement was amended to increase compensation and extend the term by Amendment N- 2002 -115, dated July 1, 2002. C. In accordance with the terms and conditions of said Agreement, the parties wish to amend said Agreement to extend the term and amend the Scope of Services. Wherefore, in consideration of the covenants contained in said Agreement, and subject to all the terms and conditions of said Agreement, except those amended in this Amendment to Consultant Agreement, the parties agree as follows: 1. Section 1, SCOPE OF SERVICES, shall be amended to read as follows: "Consultant shall perform those services as set forth in Exhibit A -1 to this Second Amendment to Consultant Agreement." 2. Section 3, TERM, shall be amended to read as follows: "This Agreement shall commence on the October 23, 2001 and terminate on June 30, 2004, unless terminated earlier in accordance with Section 12, below. The term of this Agreement may be extended upon a writing executed by the Chief of Police and the City Attorney." Work with individual inmates to establish reachable goals. 2. Keep current on community resources and refer inmates to appropriate agencies, programs or college resources. 3. Counsel individuals or small groups of inmates to train them on the best ways to make use of community resources, and what to do when obstacles occur, including but not limited to: • transportation routes • Names of specific people at known agencies • establishing a procedure for filling out forms and asking for appropriate help • providing an SOS hotline number to call in case of problems. 4. Prepare database of contacts at outside agencies, referrals advised and follow up procedures established. 5. Contact community resources to establish rapport with an individual contact person. 6. Respond to hotline calls one day per week. 7. Learn and follow all security procedures established by Jail Administration. 8. Meet with instructors and counselors on a regular basis. 9. Assist instructors as directed. 10. Report to SOS Project coordinator. 11. Keep a record of time worked during meetings and counseling sessions. CITY'S RESPONSIBILITIES: The City shall provide the following assistance to Consultant: a. Identify a contact person who shall be responsible for approving the schedule of programs and coordinating the services provided by Consultant. The subject matter, the number of hours and the scheduling of hours shall be approved by City's contact person. b. Provide an adequate facility that will allow Consultant to perform the services required by this Agreement in an efficient and timely manner.
