AMENDMENT TO AGREEMENT N- 2001 -111 _ U 1—
THIS AMENDMENT, made and entered into this 19`h day of June 2001, by and between Alfredo
Alvarado ( "Consultant") and the City of Santa Ana, a charter city and municipal corporation duly
organized and existing under the Constitution and laws of the State of G�a17,�g1 - collectively

referred to herein as "the Parties". 1 . f

A. The Parties entered into that certain agreement entitled "Consult an4 Xgreem t dated November
30, 2000, hereinafter referred to as "said Agreement ", for providing gymnastic classes at Jerome Center.
B. The Parties hereto now desire to amend the Compensation, Term and Scope of Service of said
Agreement in order to in order to provide continuous uninterrupted service under the Agreement.
WHEREFORE, in consideration of the mutual and respective covenants and promises hereinafter
contained and made, and subject to all of the terms and conditions of said Agreement as hereby amended,
the parties hereto do hereby agree as follows:
1. Section 1, of said agreement, pertaining to Scope of Services of said agreement is hereby amended
to provide no classes held on the following dates: July 4, 2001; September 3, 2001; November 12,
2001; November 22 -23, 2001; December 24, 2001- January 4, 2002; January 21, 2002; February
18, 2002; May 27, 2002.
2. Section 2, of said agreement, pertaining to "Compensation" term of said Agreement is hereby
amended to reflect an increase in compensation of an amount not to exceed $10,000.00 due to an
unexpected increase in the City's need for the Consultant's services under said Agreement.
3. Section 3, of said Agreement, pertaining to Term of said Agreement is hereby amended to change
the termination date of June 30, 2001, to June 30 2002, in order to provide necessary continuous
uninterrupted service to the Agency.
4. Except as hereinabove modified, the terms and conditions of said Agreement remain unchanged
and in full force and effect.
IN WITNESS WHEREOF, the parties hereto have executed this Amendment to said Agreement the
date and year first above written.
