AMENDMENT TO AGREEMENT FOR THE PROVISION OF SERVICES
BY EAST EDINGER MEDICAL CENTER
THIS AMENDMENT TO AGREEMENT (AMENDMENT) made and entered into
this VAl.! day of t7~J_~ 2001, by and between the City of Santa
Ana, a charter city and unicipal corporation of the State of California ("CITY") and East
Edinger Medical Clinic (VENDOR').
Recitals:
WITNESSETH
A. CITY AND VENDOR entered into an Agreement for the Provision of
Services by East Edinger Medical Clinic, dated September 24, 2000, by which
VENDOR agreed to perform medical screening services, influenza vaccines and
wellness physicals (the "AGREEMENr).
B. Pursuant to the AGREEMENT, the time for completion of the services was
June 30, 2001. The parties now desire to extend the time for completion to June 30,
2002.
WHEREFORE, in consideration of their mutual and respective promises, and
subject to the terms and conditions of the AGREEMENT as hereby amended, the
parties hereby agree as follows:
1. Section 1 of the AGREEMENT is hereby amended to read as follows:
'The services to be performed pursuant to this Agreement shall be
completed on or before June 30, 2002, unless extended in a writing
executed by the Executive Director of the Personnel Agency and the
City Attorney."
2. Except as hereinabove modified, the terms and conditions of the
AGREEMENT remain in full force and effect.
