RECITALS: A. The parties entered into Agreement N-2006-060, dated April 3, 2006 (hereinafter "said Agreement's by which Consultant has provided dance classes through the City's leisure class program. B. Consultant has incorporated her business under the name "Act One Academy of Dance, Inc.". The parties desire that the Agreement reflect the new name of the studio. C In accordance with the terms and conditions of said Agreement, the parties wish to renew said Agreemant for an additional one-year. WHEREFORE, in consideration of the covenants contained in said Agreement, and subject to all the terms and conditions of said Agreement, except those amended in this Amendment to Consultant Agreement, the parties agree as follows: 1. Consultant's business name shall be changed to Act One Academy of Dance, Inc. Payments will be made and taxes charged to the corporation. 2. Section 3, TERM shall be amended to extend the term through Jwte 30, 20 ] 0. 3. Except as hereinabove amended, all terms and conditions of said Agreement shall remain in full force and effect.
