Seman Date of Report: 01/29/2013 <br /> <br />Page 1 of 3 <br /> <br /> <br /> <br />CASE NAME: LEON, IGNACIO JR. <br />CLASSIFICATION: OFFICER INVOLVED SHOOTING (NON-FATAL) <br />AGENCY: SANTA ANA POLICE DEPARTMENT <br />DATE AND TIME: DECEMBER 16, 2012 2344 HOURS <br />LOCATION: 506 EAST WASHINGTON AVENUE SANTA ANA, CA <br />AGENCY CASE NUMBERS: SAPD: 12-36993 <br />OC CRIME LAB: 12-58683 <br /> <br /> <br />On Sunday, December 16, 2012, at approximately 0100 hours, I spoke with Santa Ana <br />Police Commander COLEMAN who advised of an officer involved shooting, which occurred at <br />506 East Washington Avenue, in the city of Santa Ana. COLEMAN requested the Orange County <br />District Attorney’s Officer Involved Incident Investigation Team, respond to the location and <br />conduct an investigation into the incident. <br />I notified District Attorney Investigators BRETT ANDERSEN, TOM CONKLIN, KORY <br />DEGRAFFENREID, PAUL CARVO, MARK GUTIERREZ, FRANK REYNOSO and ED WOOD and <br />requested they respond to assist in the investigation. I went to the shooting location and met <br />with personnel assigned to the incident. <br />On December 16, 2012 at approximately 0245 hours, Santa Ana Police Sergeant JAY <br />MILLER conducted a briefing on the known circumstances surrounding the incident for <br />personnel involved in the investigation.
