SUBJECT: In-Custody Death of Darryle Marcell Samuel Sr.
ICD-2021-0001
I have reviewed Corporal Johanna Lizardi's administrative report and associated reports related to this
investigation. Based on my review, I have come to the following conclusion:
On Friday, June 25, 2021 Correctional Officer (CO) Suzanne Tolone was assigned to the Jail as a housing officer
for Module 4C 0630-0930 hours. At approximately 1040 CO Esteban Gonzalez assumed module 4C's post and at
1118 hours inmate Darryle Marcell Samule Sr. was found unresponsive in cell 4C17.
The Orange County Sheriff-Coroner's Office determined Samuel's cause of death was ligature hanging by
suicide, which was noted, in Corporal Lizardi's administrative report.
