Kelly Approved By: <br />Date of Report: 10/23/12 Date:10/23/12 <br />Page 1 of 1 <br /> <br /> <br /> <br />CASE NAME: GUILLEN, JAIME LOPEZ <br />CLASSIFICATION: OFFICER INVOLVED SHOOTING (NON-FATAL) <br />TYPE: INITIAL RESPONSE <br />AGENCY: SANTA ANA POLICE DEPARTMENT <br /> <br /> On October 22, 2012, at approximately 2240 hours, Orange County District Attorney <br />(OCDA) Supervising Investigator RON SEMAN contacted me regarding a Santa Ana Police <br />Department (SAPD) officer involved shooting. SEMAN requested I respond to the area of 520 <br />West Walnut Street, in the city of Santa Ana, to assist with the investigation. <br /> At approximately 2340 hours, I arrived on scene and met with OCDA and SAPD <br />investigators who were present to assist with the investigation. <br /> On October 23, 2012, at approximately 0037 hours, SAPD Sergeant JIM ARMSTRONG <br />conducted a briefing of the known circumstances surrounding the incident for the personnel <br />involved in the investigation. <br /> Following the briefing, SEMAN requested I assist OCDA Investigator PAUL CARVO, who <br />SEMAN assigned as the lead investigator. <br />ORANGE COUNTY DISTRICT ATTORNEY’S OFFICE <br />BUREAU OF INVESTIGATION <br /> <br />SUPPLEMENTAL REPORT
