CONKLIN Approved By: <br />Date of Report: 10/25/12 Date: 10/26/12 <br />Page 1 of 4 <br /> <br /> <br />CASE NAME: GUILLEN, JAIME LOPEZ <br />CLASSIFICATION: OFFICER INVOLVED SHOOTING (NON-FATAL) <br />TYPE: INITIAL RESPONSE / OFFICER PROCESSING <br />AGENCY: SANTA ANA POLICE DEPARTMENT <br />On Monday, October 22, 2012, at approximately 2230 hours, Orange County District <br />Attorney Supervising Investigator RON SEMAN contacted me regarding an officer involved <br />shooting incident involving the Santa Ana Police Department. SEMAN requested I respond to <br />the intersection of Birch Street and Pine Street, in the city of Santa Ana, and assist with the <br />investigation. <br />At approximately 2337 hours, I arrived at the incident location and met with <br />investigators from the Orange County District Attorney’s Office (OCDA) and from the Santa Ana <br />Police Department (SAPD) who were present to assist in the investigation. <br />At approximately 0037 hours, Santa Ana Police Sergeant JIM ARMSTRONG conducted a <br />briefing of the known circumstances surrounding the incident for the personnel involved in the <br />investigation. <br />After the briefing, OCDA Supervising Investigator RON SEMAN requested that I conduct <br />the officer processing at the Santa Ana police station. <br />On Tuesday October 23, 2012, at approximately 0245 hours I witnessed Orange County <br />Crime Lab Forensic Specialist LINDSEY OLSON conduct processing of SAPD Officer DAVID <br />ENRIQUEZ. <br />ORANGE COUNTY DISTRICT ATTORNEY’S OFFICE <br />BUREAU OF INVESTIGATION <br /> <br />SUPPLEMENTAL REPORT
