CONKLIN Approved By: Date of Report: 10/25/12 Date: 10/26/12 Page 1 of 4 CASE NAME: GUILLEN, JAIME LOPEZ CLASSIFICATION: OFFICER INVOLVED SHOOTING (NON-FATAL) TYPE: INITIAL RESPONSE / OFFICER PROCESSING AGENCY: SANTA ANA POLICE DEPARTMENT On Monday, October 22, 2012, at approximately 2230 hours, Orange County District Attorney Supervising Investigator RON SEMAN contacted me regarding an officer involved shooting incident involving the Santa Ana Police Department. SEMAN requested I respond to the intersection of Birch Street and Pine Street, in the city of Santa Ana, and assist with the investigation. At approximately 2337 hours, I arrived at the incident location and met with investigators from the Orange County District Attorney’s Office (OCDA) and from the Santa Ana Police Department (SAPD) who were present to assist in the investigation. At approximately 0037 hours, Santa Ana Police Sergeant JIM ARMSTRONG conducted a briefing of the known circumstances surrounding the incident for the personnel involved in the investigation. After the briefing, OCDA Supervising Investigator RON SEMAN requested that I conduct the officer processing at the Santa Ana police station. On Tuesday October 23, 2012, at approximately 0245 hours I witnessed Orange County Crime Lab Forensic Specialist LINDSEY OLSON conduct processing of SAPD Officer DAVID ENRIQUEZ. CONKLIN Approved By: Date of Report: 10/25/12 Date: 10/26/12 Page 2 of 4 The following individuals were present during the processing of Officer DAVID ENRIQUEZ: • , Attorney at Law • BOB WOODING, Sergeant-Santa Ana Police Department • CHRIS REVERE, Sergeant-Santa Ana Police Department • RON SEMAN, Supervising Investigator-OCDA • PAUL CARVO, Investigator-OCDA The following items were collected and/or photographed by OLSON under FR#12-55780: SANTA ANA POLICE OFFICER DAVID ENRIQUEZ • Glock, Model 22, .40 caliber, semi-automatic handgun, serial number KEZ535, removed from the right side holster affixed to ENRIQUEZ’ Sam Brown belt. • One .40 caliber cartridge in the chamber of the handgun o Eleven .40 caliber cartridges from the magazine in the handgun (15 round capacity) • Two magazines from the left front magazine pouch of ENRIQUEZ’ Sam Brown belt containing the following: o Fifteen .40 caliber cartridges from each magazine (15 round capacity each) During the processing Officer ENRIQUEZ advised he carries his duty weapon “plus one”, meaning the seated magazine is loaded to capacity (15) with one round also in the chamber of the handgun. Officer ENRIQUEZ was dressed in full Santa Ana Police patrol uniform with a police badge above his left breast shirt pocket, and Santa Ana Police patches on both shoulders. His Sam Brown contained the following items: • Two sets of handcuffs • Taser brand X26 taser gun, one cartridge in chamber and one extra cartridge S.A. 12-026 Investigator: T. CONKLIN Approved By: Date of Report: 10/25/12 Date: 10/26/12 Page 3 of 4 •Set of keys •Safari Land Holster •Canister of Oleoresin Capsicum •Hand held police radio ENRIQUEZ was not carrying a back-up weapon. The following indicates the unaccounted cartridge count: • DAVID ENRIQUEZ 4 .40 caliber cartridges During the processing of Officer ENRIQUEZ, the following injuries from the incident were observed and photographed by OLSON: • Small laceration to right wrist • Small laceration to left palm At approximately 0258 hours, I witnessed Orange County Crime Lab Forensic Specialist LINDSEY OLSON photograph and conduct a round count of SAPD Officer ANTONIO GRAHAM. The following individuals were present during the round count: • , Attorney at Law • Sergeant BOB WOODING-Santa Ana Police Department • Sergeant CHRIS REVERE-Santa Ana Police Department • RON SEMAN, Supervising Investigator-OCDA • PAUL CARVO, Investigator-OCDA SANTA ANA POLICE OFFICER ANTONIO GRAHAM • Glock, Model 22, .40 caliber, semi-automatic handgun, serial number CWY045, removed from GRAHAM’s Sam Brown belt. • One .40 caliber cartridge in the chamber of the handgun o Fifteen .40 caliber cartridges from the magazine in the handgun (15 round capacity) S.A. 12-026 Investigator: T. His Sam Brown contained the following items: • Two sets of handcuffs • Taser brand X26 taser gun, one cartridge in chamber • Set of keys • Safari Land Holster • Canister of Oleoresin Capsicum • Hand held police radio Officer GRAHAM had the following back up weapon: • Glock, Model 27, .40 caliber, semi-automatic handgun, serial number NNG540, contained in a black holster, removed from his left ankle holster. • One .40 caliber cartridge in the chamber of the handgun o Nine .40 caliber cartridges from the magazine in the handgun During the processing of Officer GRAHAM, the following injury from the incident was observed and photographed by OLSON: • Small laceration to left palm
