HomeMy WebLinkAboutOIS2012-0006 Interview of RN by Ross_Redacted SA12-026 Investigator: ANDREW ROSS Approved By: Date of Report:10/23/12 Date:10/24/12 Page 1 of 3 CASE NAME: GUILLEN, JAIME LOPEZ CLASSIFICATION: OFFICER INVOLVED SHOOTING (NON-FATAL) INTERVIEW OF: REGISTERED NURSE WESTERN MEDICAL CENTER - SANTA ANA 1001 NORTH TUSTIN AVENUE SANTA ANA, CALIFORNIA 92705 TELEPHONE: 714-953-3500 PRESENT: DEAN FULCHER, DETECTIVE SANTA ANA POLICE DEPARTMENT DATE AND TIME: OCTOBER 23, 2012 0300-0315 LOCATION: WESTERN MEDICAL CENTER - SANTA ANA 1001 NORTH TUSTIN AVENUE SANTA ANA, CALIFORNIA 92705 On October 23, 2012, at approximately 0300 hours, I conducted an interview of Registered Nurse regarding the Santa Ana Police Department (SAPD) officer involved shooting concerning JAIME LOPEZ GUILLEN. was the trauma nurse assigned to the critical trauma team at the time GUILLEN was admitted to Western Medical Center – Santa Ana (WMCSA). Based on WMCSA’s policy, the interview was not recorded. The following WMCSA personnel were assigned to the critical trauma team: • Trauma Nurse • Emergency Room Nurse • Operating Room Nurse • Trauma Surgeon ORANGE COUNTY DISTRICT ATTORNEY’S OFFICE BUREAU OF INVESTIGATION INTERVIEW REPORT SA12-026 Investigator: ANDREW ROSS Approved By: Date of Report:10/23/12 Date:10/24/12 Page 2 of 3 • Emergency Room Doctor • Anesthesiologist • Respiratory Therapist • Radiology Technician On October 22, 2012, at approximately 2240 hours, GUILLEN arrived at WMCSA emergency room in the custody of Orange County Fire Paramedics unit ORCO 75 and Care Ambulance Service personnel. GUILLEN had sustained an apparent gunshot wound to his left leg. GUILLEN was handcuffed to a gurney and was wearing only boxer shorts. GUILLEN was conscious, talkative and appeared fidgety and restless. GUILLEN was relinquished into the custody of the WMCSA critical trauma team. GUILLEN stated he had been “shot,” but did not provide further details. GUILLEN admitted consuming alcoholic beverages that evening; however, GUILLEN denied use of narcotics. An IV was established in GUILLEN’s right wrist to provide fluids (sodium chloride). Blood was drawn from GUILLEN for testing and he was administered pain medication (Morphine). According to Trauma Surgeon examined GUILLEN and determined he had suffered an apparent gunshot wound to the left thigh. The apparent gunshot wound appeared to be a through and through wound that affected the left lateral and left anterior thigh. The entrance/exit wound was not determined. recommended that the wound be cleaned and dressed, with no further medical intervention necessary. GUILLEN’s wound was cleaned and dressed by critical trauma team personnel per instructions. GUILLEN was monitored, pending possible discharge from the hospital later that morning.
