On Sunday, June 23, 2013, at approximately 0900 hours, Orange County District
Attorney (OCDA) Supervising Investigator, MARK GUTIERREZ, contacted me regarding the
officer involved shooting of JESSICA GONZALEZ. GUTIERREZ requested I respond to the
University of California, Irvine-Medical Center (UCIMC), located 101 The City Drive South,
Orange, California, 92868, to handle the hospital scene investigation.
At 1010 hours, I arrived at the hospital, and contacted Santa Ana Police Department
(SAPD) Officer, JUAN MONTIEL #2569 in the Emergency Room of UCIMC, Douglas Hospital (DH).
MONTIEL was seated at the nurses’ station in full view of room DH 12, Bed 16, to secure the
scene for investigators.
Care Ambulance #299 transported GONZALEZ to UCIMC with MONTIEL following in his
SAPD patrol unit. Once at UCIMC, GONZALEZ was transported to Trauma Room A where she
received medical care from the following UCIMC medical staff:
• UCIMC Trauma Physician
• UCIMC Emergency Room Registered Nurse
I conducted audio digital recorded interviews of Doctor and Registered

At 0855 hours, pronounced GONZALEZ deceased. UCIMC medical staff then
transported GONZALEZ to DH12, Bed 16, to await arrival of the Orange County Sheriff’s Coroner
Department Investigator.
The UCIMC was under construction and the emergency room was being remodeled.
During this remodeling process, the Douglas Hospital set up an interim emergency room in the
former oncology treatment center. Douglas Hospital 12 was a multi-room treatment facility,
located on the first floor of the north side of the campus. Room 16, was called Bed sixteen and
was identified by a six-by-six inch orange and white placard with “16” on it; hanging from the
ceiling above the room’s opening. East of the room opening, there was also a two-by six inch
placard affixed to the wall that had “DH12(ER) Bed 16” written on it. The room was separated
from the hallway by a closed curtain and was located immediately north of the nurse’s station.
Bed sixteen’s room was equipped with a hospital bed placed in the middle of the room, a sink
and a computer. The room was clean and free from any medical debris other than what was
contained on the gurney.
At 1030 hours, SAPD Detective JEFF LAUNI arrived at the hospital. LAUNI was in
possession of an Orange Police Department Crime Bulletin of a homicide suspect, JESSICA
GONZALEZ, with an outstanding warrant (#13CF2017) for a homicide that was committed in
Orange, on June 20, 2013. The crime bulletin contained a photograph of JESSICA GONZALEZ.
At 1045 hours, LAUNI, MONTIEL and I entered the room of DH12, bed 16 and removed
the linens that covered the decedent’s head. LAUNI compared the photo on the crime bulletin
to the female lying on the hospital gurney; both appeared to match. The sheets were replaced

just as they were found and we exited the room. LAUNI advised his coworkers by telephone of
a possible identification.
At 1100 hours, Orange County Crime Lab, Forensic Specialist, Laura-Beth Camacho,
arrived to photograph the scene under the assigned forensic records # 13-48764. CAMACHO
took thirty-nine photographs of the scene and the body during the examination.
At 1126 hours, Orange County Sheriff’s Department (OCSD) Senior Deputy Coroner
Investigator, BROOKE BEASLEY, arrived to conduct a preliminary examination of GONZALEZ and
took photographs under Coroner Case # 13-02615-BB.
At 1131 hours, SAPD Detective JIM GARCIA #2409 arrived at the Douglas Hospital’s
Emergency Room.
At 1134 hours, BEASLEY, CAMACHO, MONTIEL, LAUNI, GARCIA and I entered the room.
GONZALEZ was on a hospital gurney that had been placed in the center of DH12, Bed 16’s room,
covered with linens. The gurney was placed in the room in a north/south direction. Once BEASLEY
had removed all of the linens, I observed GONZALEZ’s head on the north end of the gurney and her
feet on the south end of the gurney, near the entrance to the room.
The following items were observed on GONZALEZ’ body:
• (1) Electro Cardiogram pad affixed to left shoulder
• (3) Electro Cardiogram pads affixed to abdomen in a triangle pattern
• (1) Interosseus line inserted into the left shin
• (1) Endotracheal tube inserted into Gonzalez’ mouth and tied off with a clamp
• (1) Pulse oximeter on left forefinger
• (1) Chest tube containing blood inserted into the right side of GONZALEZ’ rib cage, tied
off with a clamp.
I observed a large scar that appeared to have been the result of a burn on GONZALEZ’s
right arm. There were multiple circular scars and small circular bruises on both of GONZALEZ’

legs. On Gonzalez’ upper left thigh, there was a vertical linear scar approximately eight inches
in length. GONZALEZ’ hands appeared dirty and covered in dried blood. GONZALEZ had a
tattoo of the words, and a

GONZALEZ had an approximate one foot open horizontal surgical incision under her left
breast. I observed eight apparent gunshot wounds, on Gonzalez’s body. Three apparent
gunshot wounds were located on GONZALEZ’s right arm; one in the forearm, and two on the
bicep. One apparent gunshot wound was observed under GONZALEZ’s right arm. One
apparent gunshot wound was observed on her left breast. On GONZALEZ’ back, I observed
three apparent gunshot wounds; one on the right shoulder, one in the middle of the back and
one on GONZALEZ’ left flank.
At 1152 hours, BEASLEY attached paper bags to both of GONZALEZ’s hands.
At 1206 hours, Traditional Funeral Services transport attendant, , arrived
at the scene and prepared the body for transportation. placed the body inside a
white body bag. At 1211 hours, the bag was sealed and tagged by BEASLEY.
At 1215 hours, the body was removed from DH12, Bed 16 and transported to the
Orange County Coroner’s Office, located at 1071 West Santa Ana Boulevard, Santa Ana.
At approximately 1250 hours, the scene was released to UCIMC, staff members.
MONTIEL received a $5.00 bill that had been collected from GONZALEZ’S chest, at the scene, by
Orange County Fire Authority Paramedic . MONTIEL gave the $5.00 bill to
BEASLEY. BEASLEY was also notified that additional currency from GONZALEZ’S person had
been secured in UCIMC’s safe by emergency room personnel.

At 1255 hours, UCI House Supervisor, , delivered $130.00 in U.S.
Currency to BEASLEY who took possession of it, counted it in my presence, and completed a
Sheriff’s Coroner Property Inventory Sheet for all currency totaling $135.00 and provided me
with a copy.
BEASLEY inventoried GONZALEZ’S personal property. BEASLEY collected the following
items and booked them into evidence at the Orange County Sheriff’s Department.
• One pair of black women’s underwear containing buttocks pads
• One pair of black, boxer type, underwear
• One pair of black, Victoria’s Secret brand, underwear
• One pair of white socks
• One beige boot
• Once black, Vans brand, tennis shoe
• One pair of blue sweat pants, cut
• One blood soaked white sweatshirt
• Two blood soaked T-shirts
• $135 dollars in U.S. Currency
