GUTIERREZ requested I respond to the area of 622 South Harbor Boulevard, in the city of Santa Ana and assist with the investigation. At 1615 hours, I arrived at the incident location and met with personnel from the OCDA and SAPD who were present to assist with the investigation. At 1631 hours, SAPD Acting Sergeant OLIVER GRANT conducted a briefing of the known circumstances surrounding the incident for personnel involved with the investigation. Following the briefing, GUTIERREZ directed me to team with SAPD Detective JULIAN RODRIGUEZ and conduct witness interviews. RODRIGUEZ and I conducted an audio digitally recorded interview of: • At approximately 1800 hours, I responded to the Santa Ana Police Department and witnessed Orange County Crime Lab Forensic Specialists LINDSEY OLSON and AIDE SAHAGUN process SAPD Officers JESSICA GUIDRY and STEPHEN CHAVEZ under FR# 13-50719. Santa Ana Police Officer JESSICA GUIDRY Officer GUIDRY’s uniform and equipment were photographed. GUIDRY was dressed in full police uniform with a complete Sam Browne duty belt. GUIDRY was identified as firing her pistol in the ORANGE COUNTY DISTRICT ATTORNEY’S OFFICE BUREAU OF INVESTIGATION SUPPLEMENTAL REPORT S.A. 13-017 Investigator: LITCHENBERG Approved By: Date of Report: 08/12/13 Date: 10/03/2013 Page 2 of 3 shooting incident, her pistol was examined and her rounds were counted. GUIDRY’s pistol was collected as evidence. GUIDRY’s Sam Browne duty belt contained the following items: • Radio, left side • Baton ring • Two handgun magazines in a pouch on left side • Glock handgun, in a holster on right hand side • Handcuffs • Keys • Taser brand X26 Taser gun, one cartridge in chamber After photographing GUIDRY, SAHAGUN photographed and collected the following items: • Glock, Model 21, .45 caliber semi-automatic pistol, serial number CUN779, with a Stream light M3 flashlight mounted beneath the barrel. GUIDRY stated she carried her pistol with the magazine loaded to capacity (13), plus one cartridge in the chamber. • One .45 caliber cartridge from the chamber of the pistol. Twelve .45 caliber cartridges from the magazine in the pistol (13 round capacity) • Two magazines from the left front magazine pouch of GUIDRY’s Sam Brown belt containing the following: Thirteen .45 caliber cartridges from each magazine (13 round capacity each) GUIDRY was not carrying a back-up weapon. Santa Ana Police Officer STEPHEN CHAVEZ Officer CHAVEZ’ uniform and equipment were photographed. CHAVEZ was dressed in full police uniform with a complete Sam Browne duty belt. CHAVEZ was not identified as firing his pistol in the shooting incident, his duty and backup pistols were examined and a cartridge count was conducted for each. • Glock, Model 22 .40 caliber, semi-automatic pistol, Serial Number: CWY009 • One magazine from the pistol containing 15 cartridges, and 1 cartridge from the chamber of the pistol.
