Monday, March 11, 2019, 0900 hours:
On the above date and time, I received a call from Sergeant J. Rodriguez #2836 of the Santa Ana Police Department
Homicide Unit, he stated an Officer Involved Shooting had just occurred at 1800 S. Evergreen in the City of Santa
Ana, County of Orange. Sergeant J. Rodriguez told me to respond to the scene an assist with the investigation.
2220 hours:
I arrived at the scene and met with Homicide Detectives along with the Orange County District Attorney Investigators.
2245 hours:
I checked into the crime scene with Officer J. Peralta #3602 who was the scribe at the scene.
2325 hours:
Sergeant O. Lizardi #2629 of the Santa Ana Police Department provided briefing to the Santa Ana Police Department
Homicide Unit, Santa Ana Police Department Crime Scene Investigations and the Orange County District Attorney
Investigators.

Sergeant O. Lizardi stated the Santa Ana Police Department Gang Unit along with the Orange County Probation
Department were conducting gang suppression in the area of 1800 E. Evergreen. During this time an Officer Involved
Shooting occurred. Sergeant O. Lizardi stated Detective E. Solorio #3249 was involved in the shooting. Detective E.
Solorio provided a safety statement indicating he was involved in the shooting and no other suspects were
outstanding.
At the conclusion of the briefing, I was assigned to partner up with OCDA Investigator P. Montez and respond to OC
Global Medical Center located at 1001 N. Tustin Ave., in the City of Santa Ana to make contact with the suspect.
Tuesday, March 2, 2019, 0005 hours:
Investigator P. Montez and I contacted Officer J. Peralta to advise him we would be checking out of the crime scene
and responded to OC Global Medical Center.
0015 hours:
We arrived at OC Global Medical Center.
0025 hours:
We made contact with Registered Nurse who stated the suspect Oscar Alvarado Penaloza DOB:
05/16/96 with Acct: , MRN: arrived to the ER with a gunshot wound to the left forearm. She
said the gunshot wound was a through and through injury. She stated the suspect was complaining of pain and told
them he was taking medication for schizophrenia. told us Alvarado was taken to the Operating Room because
they could not find a pulse. At this time, walked us to the OR waiting area were we met with Officer¶s Liggett
#3488 and Henderson #3518.
0147 hours:
Vascular Surgeon came out of the surgery and stated the surgery was complete. He stated Alvarado Penaloza
sustained a broken bone on his left arm and nerve injury.
Alvarado Penaloza was taken to the recovery room where he remained until he was transported to CCU bed #16.
Alvarado Penaloza was administered a sedative to help him relax.
0350 hours:
Officers Phan #3414 and Meyers #3389 relieved Officers Liggett #3488 and Henderson #3518. Officers Phan and
Meyers remained at the hospital with suspect Alvarado Penaloza.
0430 hours:
Orange County Sheriff-Coroner Department Lead Forensic Specialist L. Pena-Sanchez arrived to the hospital to take
photos and collect the clothing of suspect Alvarado Penaloza.
0445 hours:
Investigator Montez and I cleared OC Global Medical Center.
