EVIDENCE

SCENE
The incident involving Washington took place inside an elevator within the Santa Ana City Jail, located at 62 Civic
Center Plaza in Santa Ana.
Detox Cell "D1"
Prior to being taken to the elevator, Washington was detained in detox cell "D1," situated on the first floor, just east of
the booking area.
•Surrounding Area:
•A large plastic trash bin, marked with yellow police tape, extended westward from D1 toward two rows of plastic
chairs positioned west of the cell.
• The police tape encircled a mobile restraint chair near the unisex bathroom door and extended to a water
fountain located north of the bathroom.
•Restraint Chair Details:
•Constructed of a gray metal frame with a cushioned plastic seat and backrest.
•Designed with small rear wheels, a metal footrest, and handlebars covered in rubber grips.
•Black seat belt-style straps were woven through the handles and secured with metal buckles under the
armrests.
•Additional restraint straps were located above the footrest, between the backrest and seat, and at the top of the
backrest.

• Three windows (3 ft by 2.5 ft) lined the upper portion of the west wall, with an additional window (3 ft by 2 ft)
integrated into the cell door, allowing direct visibility into the interior.
•Interior of D1:
•The cell featured white cement flooring and walls, along with two 3-ft partition walls for bathroom privacy.
•Fixtures included:
•A white sink mounted on the north wall, approximately 3 ft above the floor.
•A white toilet positioned east of the sink, accompanied by a metal toilet paper holder affixed to the wall.
•A drainage hole located on the floor southwest of the toilet.
•A roll of toilet paper placed on the floor east of the toilet.
•A cord-free telephone mounted on the southeast wall.
•A metal intercom attached to the south wall.
•A dome surveillance camera positioned on the ceiling above the east wall.
Elevator Description
Washington was escorted from D1 to an elevator west of the booking area, past a set of security doors. Three
elevators are aligned along the north wall, with Washington being placed in the center one.
•Exterior Features:
•The elevator doors were made of silver steel, with the inscription “D63931 SAN 5208 ELEV. #2-H” located on
the metal frame above the door.

•Dimensions: Approximately 6 ft by 6 ft.
•Walls: Silver steel, with handrails mounted 3 ft 10 inches above the floor.
•Controls:
•A button panel on the south wall indicated floors ranging from the basement to the fourth level.
•A white plastic fob reader was positioned above the keypad.
•Above the controls, a black digital screen displayed the current floor number, along with a framed permit for the
elevator.
• Surveillance Camera: Mounted in the northeast ceiling corner.
At approximately 4:30 PM, personnel from the OCCO and OCCL conducted the scene examination under OCCL
forensic report #25-40575. The following individuals participated in processing the scene:

SUMMARY
On 1/15/25, at 1745 hours, arrestee Freddie Washington DOB 2/22/1988, arrived to the Santa Ana Jail under arrest
and uncooperative. Washington arrived in the maximum restraint device (The Wrap.) Washington was removed from
the police vehicle and placed in the maximum restraint chair by Santa Ana Police jail staff. At 1752 hours, Washington
was medically evaluated by jail medical staff and admitted to the jail. He was placed in Detox cell 1, until 1/16/25.
At 0157 hours, Washington was removed from Detox cell 1 but remained in the maximum restraint chair while he was
processed by jail staff. Washington was medically evaluated two more times at 0234 hours, and 1326 hours. At 1329
hours, Washington was in medical distress and later transported to St Joseph’s hospital where he later died at 1438
hours.
INVESTIGATION
On Thursday, January 16th 2025, I was at my desk at the Santa Ana Police Department when Detective Sergeant M.
Lemmon #3174 notified me of an in-custody death that occurred at the Santa Ana Jail. I responded to the jail and
assisted with the investigation
1613 HOURS:

Correctional Supervisor E. Villegas (#3098) conducted a briefing with detectives from the Santa Ana Police
Department Homicide Unit, inspectors from the Professional Standards Unit, and investigators from the Orange
County District Attorney’s Office. The briefing focused on the actions of the correctional officers involved in the
investigation. Below is a timeline of events.
Timeline of Events:
· January 15, 2025, 1745 hours: Freddie DeAngelo Washington (DOB 2/22/88) arrived at the Santa Ana Police
Detention Facility’s sally port. Due to his uncooperative behavior, he was placed in a restraint chair with a spit mask.
· 1752 hours: Jail medical staff assessed Washington in the pre-booking area.
· 1756 hours: Washington was moved to detox cell D1, and the spit mask was removed.
· 2104 hours: Senior Correctional Officer E. Salgado (#3353) attempted to allow Washington to stretch his legs.
Washington did not respond but was awake and conscious.
· January 16, 2025, 0157 hours: Washington, still in the restraint chair, was moved to inmate processing by
Senior Correctional Officer C. Paulino (#3368). The inmate processing interview and booking photos were completed,
but Washington refused to provide latent prints or a DNA sample.
· 0205 hours: Washington was returned to detox cell D1 while still in the restraint chair.
· 0234 hours: Senior Correctional Officer Salgado re-assessed Washington. A medical evaluation was offered
and accepted. Washington's vitals and restraint straps were checked, but he refused to stretch his arms.
· 0616 hours: An inmate headcount was conducted.
· 1048 hours: Correctional Officers C. Carrillo (#3918) and J. Alston (#3663) offered Washington water and a
chance to stretch. He accepted the water.
· 1316 hours: Washington was removed from the restraint chair, handcuffed, and seated in the intake/booking
area.
· 1326 hours: He was medically cleared by Registered Nurse for transport to the Orange County
Jail, along with another arrestee,
· 1328-1329 hours: As they entered elevator H2, Washington suddenly fell backward, appearing to faint or have
a seizure. Officers Carrillo and Alston guided him to the ground, and medical assistance was requested.
· 1330 hours: Nurse provided medical aid.
· 1333 hours: Washington regained consciousness but became uncooperative.
· 1334 hours: He was placed back in a restraint chair with a spit mask applied, remaining conscious and
breathing.
· 1338 hours: During a medical evaluation in detox cell D1, it was discovered that Washington was not breathing.
He was immediately removed from the chair, and CPR was administered.
· 1340 hours: The Orange County Fire Authority was called for medical aid.
· 1347 hours: Medics arrived and continued life-saving measures.
· 1417 hours: Washington was transported by ambulance to St. Joseph’s Hospital.
· 1438 hours: He was pronounced deceased by the emergency room doctor.
Investigation Assignment:
Following the briefing, Santa Ana Police Department personnel provided a walkthrough of the jail where the incident
occurred. I was assigned to document the scene, with Orange County District Attorney’s Office Investigator C.
McCarthy.
At the conclusion, I released the scene to SAPD jail staff and returned to my office. This concluded my involvement in
Santa Ana PD 2025-01082 6 of 7

On Thursday, 1/16/2025 at approximately 1430 hours, I was dispatched to 62 Civic Center Plaza (Santa Ana Jail) in
reference to a medical aide that occurred. I was assigned to interview a witness at the Santa Ana Jail who saw the
incident. While at the Jail, I interviewed an inmate who was later identified as a ). The
following is a brief summary of 's statement:
described the decedent to be a male black, wearing blue shorts, black shirt and white slippers. first
saw the male as he was walking to the elevator inside of the Santa Ana Jail. She stated the male appeared to be
dizzy as he was wobbly walking towards the elevator. stated the male was in front of him inside of the
elevator as it traveled down. There was two correctional officers, her and the male inside of the elevator.
stated the male collapsed in front of her in the elevator. stated the correctional officers assisted the male
after he collapsed.

On 1/16/24, I was working in the capacity of a sworn Police Officer. At approximately 1740 hours I was dispatched to
1100 W Stewart Drive in The City of Orange to assist with an investigation.
I arrived at the hospital at approximately 1745 took control of scene security in front of emergency room #9. I
temporally took custody of the patients property before relinquishing it over to the Orange County Coroner on scene. I
left the hospital at approximately 1830 hours.

THURSDAY, JANUARY 16, 2025:
While at my desk, Detective Sgt. M. Lemmon #3174 notified me of an in-custody death investigation. He requested I
respond to the Santa Ana Police Jail and assist with the investigation.
1615 HOURS:
Santa Ana Police Correctional Supervisor E. Villegas #3098 briefed members of the Santa Ana Police Homicide Unit,
Santa Ana Police Professional Standard Inspectors, Orange County District Attorney Investigators and Orange
County Sheriff’s Forensic Unit personnel.
Supervisor Villegas advised Santa Ana Police Correctional Officers were escorting the decedent, identified as Freddie
Washington, and another inmate to the Orange County Jail. On the walk to the Orange County Jail, Washington
collapsed.
Washington was transported to St. Joseph Hospital where he was later pronounced deceased.
At the end of the brief, Santa Ana Police Correctional personnel walked us through the jail where the incident
occurred.
1640 HOURS:
Correctional Manager K. Willard #2106 provided me with a flash drive of footage of the incident. A second copy was
made, which I booked into evidence. The original flash drive was given to Orange County District Investigators.

1/16/2025 1615 hours:
While in the Santa Ana Detention Facility roll call room, Santa Ana Detention Supervisor E. Villegas #3098 briefed
members of the SAPD Homicide Unit, Orange County District Attorney's Office, Orange County Sheriffs Forensic Unit,
and SAPD Internal Affairs Unit regarding an in- custody death. Supervisor E. Villegas provided us with the decedents
name (Freddie Washington), an arrestee ( ) who was present when Freddie fell to the ground,
the two Detention Officers that were in the process of escorting the inmates to Orange County Jail, and the timeline
from the decedents arrival to his time of death.
At the conclusion of the brief, I was assigned to interview along with District Attorney Investigator
Claudia McCarthy.
1623 hours:
We did a walkthrough of the first floor where the incident occurred.
1638 hours:
Investigator Claudia McCarthy and I contacted a , while she was in holding cell #H-3. a was a
Spanish speaker. Investigator McCarthy and I are both certified Spanish speakers. After introducing ourselves to
a, we spoke to her in an interview room in the booking portion of the jail. a was informed that we wanted to
only speak to her about the incident with Freddie. a told us the following:
a was arrested the previous night and placed in a holding cell next to Freddie. a did not know Freddie by
name, but described him as a black male, wearing blue shorts, and slippers. a did not hear or see anything
unusual throughout the night. a had a clear view of Freddie as she was able to see him through the glass
window.
Today (1-16-2025), a was escorted out of her holding cell and sat in a chair in the open door room (booking area)
of the jail facility. a was handcuffed with her hands behind her back. Freddie sat a few feet away from her.
Freddie was also handcuffed, but a did not recall if he was handcuffed to the front or back. a saw that
Freddie sat quietly in his chair. a and Freddie sat for approximately 5 minutes. Two Detention Officers (Carrillo
#3918 and Austin #3663) approached Freddie and a and were ordered to get in a single file line. A Detention
Officer was next to each inmate. As they were about to enter the elevator, Freddie entered first and a walked
behind him. a heard Freddie say that he was "dizzy". a reached the back left corner of the elevator and
stared at the wall. Freddie, who was to her right, suddenly fell backwards. a looked to her right and saw Freddie
face up. The Detention Officers began rendering aid. Due to the elevator door closing, the Detention Officers pulled
Freddie outside the elevator. a was eventually escorted back to a holding cell.
We informed a that Freddie had passed. a was surprised because she did not see anything unusual with
Freddie. We then ended our interview with a.

On 1/16/25 at approximately 1343 hours, I was dispatched to 62 Civic Center Plaza (Santa Ana Jail) in reference to a
medical aid. Upon arrival, I observed Orange County Fire Authority performing CPR on a black male (Freddie
Washington) who was lying face up on the floor near the holding cells.
OCFA stated they would be transporting Washington to a hospital. I was tasked to follow the ambulance. Prior to
leaving the Santa Ana Jail, I was provided and took custody of Washington's property which was in a sealed clear
property bag. The property included Washington's New York driver's license along with a property receipt. At
approximately 1417 hours, Falck ambulance #418 left the Santa Ana Jail and arrived at 1100 W Stewart Drive (Saint
Joseph Hospital) at approximately 1426 hours. Washington was admitted to emergency room #9. At approximately
1438 hours, medical staff pronounced Washington deceased.
I took control of scene security in front of emergency room #9.
At approximately 1742 hours, I relinquished Washington's property and scene control to Corporal Engle and left St.
Joseph Hospital.

On the above date and time, I responded to the Santa Ana Jail reference an inmate (WASHINGTON, Freddie)
suffering a medical emergency. After arriving, WASHINGTON was transported by ambulance to St. Joseph's Hospital
in the city of Orange. I later responded to the hospital and Officer MERCADO advised me WASHINGTON was
pronounced deceased at 1438 hours.

Thursday, January 16, 2025:
While at my desk, Detective Sergeant M. Lemmon #3174 notified me of an in-custody death investigation. He
requested I respond to the Santa Ana Police Jail and assist with the investigation.
1615 hours:
Santa Ana Police Correctional Supervisor E. Villegas #3098 briefed members of the Santa Ana Police Homicide Unit,
Santa Ana Police Professional Standard Inspectors, Orange County District Attorney Investigators, and Orange
County Sheriff Forensic Unit personnel.
Supervisor Villegas said Santa Ana Police Correctional Officers Freddie Washington when he collapsed. Washington
would be transported to St. Joseph’s hospital where he would be pronounced deceased.
1623 hours:
Correctional Supervisor Villegas ended the briefing. Santa Ana Police Correctional personnel walked us through the
jail where the incident occurred.
I was later tasked with responding to St. Joseph's Hospital along with District Attorney Investigator J. Hoover.
1635 hours:
I left the Santa Ana Jail and responded to St. Joseph's Hospital.
1706 hours:
I arrived at St. Joseph's Hospital and met with Investigator Hoover and Orange County Sheriff's Deputy Coroner L.
Martin ##11543.
1743 hours:
Investigator Hoover and I entered St. Joseph's Hospital through the emergency room entrance and contacted
Corporal T. Engle #3629 and Sergeant B. Mercer #3077, standing outside Emergency Room #9. Corporal Engle
advised Washington was inside Emergency Room #9. The entry to the room consisted of a sliding glass door with an
extended curtain on the interior. The curtain prevented a visual inside the room.
1746 hours:
Orange County Forensic Specialist L. Perlin #11846 arrived. Investigator Hoover, Forensic Specialist Perlin, Deputy
Coroner Martin, and I entered Emergency Room #9. Deputy Coroner Martin and Forensic Specialist Perlin took
photographs and conducted an initial examination of Washington’s body. Deputy Coroner Martin collected
Washington's clothing and property which included his New York identification ). The
identification contained Washington's photograph and the following information:

Deputy Coroner Martin concluded her examination of Washington's body and placed a toe tag on his right toe.
1829 hours:
Washington's body was placed inside a body bag that was sealed. The seal used for the body bag contained seal
#0236599.
1831 hours:
Washington's body was transported to the Orang County Sheriff's Coroner's facility by Traditional Funeral Services.
I cleared the hospital along with Investigator Hoover. Detective C. Shynn #3238 was designated as the case agent for
this incident.

Thursday, January 16, 2025:
While at my designated workstation at the Santa Ana Police Department, Detective Sergeant Homicide Section
Detective Sergeant M. Lemmon #3174 notified me of an in-custody death investigation. Sergeant Lemmon requested
that I respond to the Santa Ana Detention Facility in order to assist with the investigation.
1615 hours:
Santa Ana Police Department Correctional Supervisor E. Villegas #3098 conducted a briefing with detectives from the
Santa Ana Police Department Homicide Unit, inspectors from the Santa Ana Police Department Professional
Standards Unit, and investigators from the Orange County District Attorney’s Office, regarding the actions of the
correctional officers involved in the investigation. The following is a timeline of the events provided by Supervisor
Villegas.
On January 15, 2025, at 1745 hours, Freddie DeAngelo Washington (DOB 2/22/88) arrived at the police vehicle sally
port of the Santa Ana Police Detention Facility. Washington was uncooperative with patrol officers and correctional
officers. From the sally port, Washington was placed in a restraint chair with a spit mask.
At 1752 hours, jail medical staff assessed Washington in the pre-book area.
At 1756 hours, Washington was placed in detox cell D1 and the spit mask was removed.
At 2104 hours, Senior Correctional Officer E. Salgado #3353 attempted to provide Washington a chance to stretch his
legs. Washington refused to respond to Senior Correctional Officer Salgado. Washington was conscious and awake
during this interaction.
On January 16, 2025, at 0157 hours, Washington remained in the restraint chair and was wheeled to inmate
processing by Senior Correctional Officer C. Paulino #3368. An inmate processing interview and booking photos were
completed. Washington refused to provide latent prints or a DNA sample.
At 0205 hours, Washington remained in the restraint chair and was placed back into detox cell D1.
At 0234 hours, Senior Correctional Officer Salgado re-assessed Washington. Medical evaluation was offered and
accepted. Washington's vitals were evaluated and the straps to the restraint chair were evaluated. Washington
refused to stretch out his arms.

At 0616 hours, an inmate headcount was conducted.
At 1048 hours, Correctional Officer C. Carrillo #3918 and Correctional Officer J. Alston #3663 offered to allow
Washington to stretch his limbs and drink water. Washington accepted the water.
At 1316 hours, Washington was removed from the restraint chair. He was then handcuffed behind his back and
seated in a chair in the intake/booking area.
At 1326 hours, Washington was medically cleared by Registered Nurse for transport to the Orange
County Jail. Another arrestee, identified as a ), was grouped with Washington for
transportation to the Orange County Jail.
At 1328 hours, Washington and were escorted by Correctional Officers Carrillo and Alston to the elevators.
At 1329 hours, they all entered elevator H2 and Washington suddenly fell backwards in an apparent fainting spell or
seizure. Correctional Officers Carrillo and Alston helped guide Washington to the ground. Washington was then
removed from the elevator for medical assistance.
At 1330 hours, Nurse responded to provide medical aid to Washington.
At 1333 hours, Washington suddenly regained consciousness and became uncooperative with correctional officers
and medical staff.
At 1334 hours, Washington was placed back into a restraint chair and a spit mask was applied. Washington was
conscious and breathing.
At 1338 hours, Washington remained in the restraint chair and he was wheeled to detox cell D1 for further medical
evaluation. During the medical evaluation, it was discovered that Washington was not breathing and he was
immediately removed from the restraint chair. CPR was then performed on Washington.
At 1340 hours, Orange County Fire Authority was requested for medical aid for Washington.
At 1347 hours, medics from the Orange County Fire Authority arrived on scene. They continued life saving measures
on Washington.
At 1417 hours, Washington was transported to St. Joseph’s Hospital by ambulance.
At 1438 hours, Washington was pronounced deceased by the treating emergency room doctor at St. Joseph’s
Hospital.
At the conclusion of the briefing, Santa Ana Police Department Correctional personnel walked us through the jail
where the incident occurred. After observing where the incident occurred, I was assigned as the lead investigator from
the Santa Ana Police Department for this investigation. Orange County District Attorney’s Office Investigator T. Toth
was assigned as their lead investigator.
Santa Ana Police Department Detective Corporal B. Speakman #3360 was assigned to document the scene of where
the incident occurred with Orange County District Attorney’s Office Investigator C. McCarthy. For further details, refer

Santa Ana Police Department Detective F. Gomez #2706 was assigned to collect surveillance video of the
incident. For further details, refer to Detective Gomez’s supplemental report under this case number.

County District Attorney’s Office Investigator McCarthy. For further details, refer to Detective Capacete’s supplemental

Santa Ana Police Department Detective G. Corona #3319 was assigned to respond to St. Joseph’s Hospital with
Orange County District Attorney’s Office Investigator J. Hoover to document the processing of Washington’s body. For

I returned to the Santa Ana Police Department and continued the investigation.

Thursday, January 16, 2025:
Santa Ana Police Department Homicide Unit Sergeant M. Lemmon #3174 notified detectives from the Homicide Unit
of an in-custody death investigation. Sergeant Lemmon requested the Homicide Unit respond to the Santa Ana
Detention Facility to assist with the investigation.
1615 hours:
Santa Ana Police Correctional Supervisor E. Villegas #3098 conducted a briefing with detectives from the Homicide
Unit, inspectors from the Internal Affairs Unit, investigators from the Orange County District Attorney ’s Office and
personnel from the Orange County Sheriff Forensic Unit.
Correctional Supervisor Villegas advised Santa Ana Police Correctional Officers were escorting the decedent,
identified as Freddie Washington, and another inmate to the Orange County Jail. Washington collapsed during the
escort and was ultimately transported to St. Joseph Hospital where he was later pronounced deceased.

Correctional Supervisor Villegas concluded his briefing. A walkthrough of the jail where the incident occurred was
conducted.
At the conclusion of the walkthrough, Detective C. Shynn #3238 was assigned as the case agent. I was tasked with
responding to the St. Joseph Hospital with OCDA Investigator T. Hoang to interview hospital staff.
1635 hours:
I left the Santa Ana Detention Facility and responded to St. Joseph Hospital.
1706 hours:
I arrived at St. Joseph Hospital and met with Investigator Hoang. Investigator Hoang and I were tasked with
interviewing medical staff that provided medical aid to Washington.
1725 hours:
Investigator Hoang and I interviewed Dr. . Dr. is an Emergency Medical Physician at St. Joseph
Hospital. Dr. was the doctor that received Washington when he arrived at the hospital. Dr. described
the medical care she provided to Washington prior to him being pronounced deceased. For further details regarding
the interview with Dr. , refer to Investigator Hoang’s report.
1737 hours:
Investigator Hoang and I interviewed Nurse . Nurse is a nurse assigned to the Emergency
Department at St. Joseph Hospital. Nurse was the primary nurse assigned to Washington when he arrived at the
hospital. Nurse described the medical care he provided to Washington prior to him being pronounced deceased.

I left the hospital and returned to the Santa Ana Police Department.
Friday, January 17, 2025 – 0900 hours:
I attended the postmortem examination of Freddie Washington along with Orange County District Attorney’s Office
Investigator J. Hoover. I arrived at the Orange County Sheriff’s Department – Coroner’s Division located at 1071 W.
Santa Ana Boulevard in the City of Santa Ana. I entered observation room #2, which was adjacent to where the
postmortem examination would be performed. There was an approximately 3’ by 9’ window to view the examination.

OCDA Investigator Hoover briefed Dr. on the circumstances of Washington ’s death. Present in the exam room
was Forensic Assistants V. Ledesma and P. Esquivel, Forensic Scientist K. Brown and Forensic Specialist J. Guevara.
I observed Washington’s nude body in a supine position on top of the metal examination table. Washington had
several pieces of medical equipment attached to his body.
1104 hours:
Dr. concluded the postmortem examination. For further details regarding the examination, refer to Dr.

July of 2024. currently works Wednesdays – Fridays from 0600 – 1830 hours. Today, at approximately 1328
hours, checked Freddie Washington’s vitals which were normal. At that time, Washington showed no signs
of distress. Shortly thereafter, heard a medical aid request via radio near the elevator. responded
and observed Washington handcuffed on the floor. described Washington as being aggressive, fighting and
combative. Washington was placed onto a restraint chair and taken to a holding cell. checked Washington
again and noticed he was not responsive and had no pulse. Washington was given Narcan and custody personnel
provided CPR. Orange County Fire Authority arrived and Washington was transported to the hospital.
