By the end of the sixth month, clients will maintain sobriety and be in an individualized program of training or <br />employment. <br />By the end of the first year of support, at least 10 clients will be trained and ready to work or be gainfully <br />employed, or in school. <br />Every client contract will be reviewed monthly for progress, drug testing will occur as specified in the individual <br />contracts, and a progress report will be available to CDBG in month six and with a final progress review at the <br />end of the first year. Second year services will continue as funding and time commitments of both client and <br />Second Chance Orange County personnel. <br />Outcome 1: Record of communications and Memoranda of Understanding on file for each partner agency <br />Outcome 2: File of materials and attendees at trainings, and written evaluation of the partnership effort at the <br />end of training by those participating <br />Outcome 3: Record of referrals received, case manager intake process on file for all those interviewed, with <br />ten participants selected based on intake evaluation <br />Outcome 4: Introduce SCPCSA to Santa Ana Chamber of Commerce, and to various Hispanic non -profits and <br />business associations to assist and support Hispanic clients (Our Director is Hispanic, and speaks fluent <br />Spanish) Record of all outreach efforts designed to identify supportive businesses who can employ clients <br />Outcome 5: Each client file will show the connections made to address specific needs, and while data is <br />confidential, the periodic reports will enumerate the individual actions completed on behalf of each client. All <br />therapies will be documented, and all trainings will be tracked in client database. <br />Outcome 6: Employment records, pay stubs (pay stubs are routinely collected from all clients). <br />Outcome 7: Phone and meeting records, and intake process interviews with at least potential clients, by the <br />end of year one. <br />Outcome 8: Record of drug testing as contracted, when needed we ask for. attendance record at sobriety <br />programs (NA, AA). Weekly mentoring reports available <br />Outcome 9: Clear record for each client served. <br />Outcome 10: Progress reports will show numbers of clients in current training or gainfully employed as <br />specified in anticipated outcome. <br />Outcome 11: Analysis of all outcome data and preparation/distribution of half year and annual report. <br />Exhibit A <br />
