ARTS <br />2215 N Broadway - Santa Ana, CA 92706 • 714.547.5468 • Fax 714.564.9690 • www.occtac.org <br />501(c)3 Federd Tax ID# 33-0930891 <br />Fingerprinting —Live Scan and TB Test: <br />Participants who work with children and are placed in an educational setting will be <br />required to undergo Fingerprinting —Live Scan through the Department of Justice <br />and show a cleared TB Test. This supportive service will be available for participants <br />on a case -by -case basis and determined by the case manager. <br />Books and/or Uniforms —Work Related Equipment <br />A participant may need to purchase books, skill building tools or work related <br />equipment to fidly participate in returning to school and or a new job opportunity. <br />All Funding will be allocated for specific needs on a case -by -case basis, i.e. Working <br />Wardrobe —Interview outfit. <br />Portfolios -Resume -Business Cards Related items for Job Search Tools <br />Participants may put together a professional portfolio of work demonstrating their <br />skills in the area of marketing, graphic arts and design related to Job -Search. <br />Child/Dependent Care <br />After identifying the need for assistance and eliminating other resources, the case <br />manager and the participant will develop a service plan and update progress in the <br />case notes. <br />Childcare assistance is defined as the provision of $50 payment to <br />institution/daycare/childcare provider upon receiving aninvoice. <br />Participants may receive assistance with childcare or dependent care, which will <br />enable them to participate in activities. Childcare will be limited to a maximum of <br />$50.00 per month per child/dependent. Supportive service available to all youth that <br />enrolled in the program as pregnant or parenting youth. <br />Other: Any other miscellaneous type expenses not mentioned above may be considered with <br />proper authorization.
