SCOPE OF SERVICES <br />GENERAL ENGINEERING (OPTION 6) <br />Prehrninary Design <br />A. KICKOFF MEETING <br />Prior to the start of the project, we will have a kick-off <br />meeting with City Staff and the project team to review the <br />project objectives, schedule, the $cops and approach. <br />At this meeting, we will establish a point of contact for <br />the project and requestall available Information related <br />to the project. In addition, the NV5 Project Manager will <br />coordinate all aspects of the project and menage the <br />schedule and costs to ensure the projects completion to <br />the satisfaction of the City. <br />6, DATA COLLECTION AND PROJECT <br />DOCUMENTATION <br />We will research all information pertinent to the project <br />such as the latest street improvement plans, right of way <br />maps, previouslntersections and median Improvement <br />plans, storm drain; water and sewer plans, and utility plans <br />inorder to Ineludo this information in the project plans. <br />C. DESIGN SURVEY <br />The survey tasks include, but are not limited to, the <br />following; <br />We will perform a topographic survey and prepare base <br />Maps. We will snake sure our base maps show the <br />right of way, curb and gutter, sidewalk, parkway, street <br />centerline, access ramps, driveways, signs, poles, otc. <br />In addition, we will include all information obtained from <br />ail affected utility companies within the roadway and <br />parkways. The base reaps will then be confirmed during <br />our field walks, The main objective is to have all existing <br />Improvements accurately depicted In our base maps so <br />thot.all constraints are considered during our design of the <br />project. The survey will Include cross sections the street to <br />adequately show the condition of the project site prior to <br />construction, <br />D. U"fILITY INVESTIGATION AND COORDINATION <br />We will research and establish record location of all <br />utilities and utility easements, as necessary, Utility <br />coordination on the project will be ongoing and will <br />generally consist of the following sub tasks: <br />Notify each utility purveyor, in writing, of the City's <br />proposed plans and sohedule and request copies <br />of record drawings or atlas maps of existing and <br />proposed facilities within the project boundary. <br />• Obtain existing dry and.wat utility plans/records within <br />the project limits. It also Includes the coordination of <br />the adjustment of any utilities per city or other agency <br />standards. Once the plans are completed we will send <br />them to ail utility companies affected by the project. <br />• Coordinate all utilities and obtain approval in writing <br />from affected utility companies regarding conflicts, <br />relocations and improvements. <br />• Access the Underground Service Alert's website and <br />compile a list of member agencies that own facilities <br />within the project limits. <br />• Augment and/or refine the above list based on <br />available information at the City Records Department <br />and from field investigation. <br />• Utilities shown on the record drawings will be surface. <br />verified, and added to the base map, along with the <br />most recent easement lines shown for each fecJ11% <br />• Identify interfaces between existing facilities and <br />future project construction. <br />+ Consult with the. service planners and others as <br />necessary within each utility company to resolve any <br />conflicts.. <br />• Keep City staff informed on all discussions with utility <br />companies via written meeting minutes or records of <br />telephone conversation. <br />Prepare a Utility Notification Log to track utility <br />company contacts and responses including contact <br />Information and dates of ell outgoing and incoming <br />correspondence, <br />E. PAVEMENT EVALUATION <br />We will provide geotoehnical investigation, <br />recommendations, and reports to determine the exact <br />rehabilitation methods Including but not to limited to <br />reconstruction areas and the proper pavement thickness <br />for reconstruction areas and overlay. We will perform <br />a geoteehnlcal reconnaissance of the site and take up <br />to six borings samples at strategic locations within the <br />project area to observe the existing pavement thickness <br />and soil conditions beneath end to facilitate collection of <br />sub -grade soil samples for lab testing. The results of our <br />field and laboratory testing will be provided in a report <br />which will include; thickness of pavement section, site soil <br />characterization, R-Value of sub -grade soils, and pavement <br />improvements recommendations. <br />F. PRELIMINARY PLANS <br />Utiiizingthe above data and base plans, we will prepare <br />a preliminary street Improvement plains and profiles <br />using :1." = 40` horizontal and 1" = 4' vertical scale. in <br />
