City of Santa Ana <br />Situations which come to the attention of, or are reported to, the OCFA EMS <br />Section Battalion Chief and which appear to constitute a Contractor service <br />or performance deficiency or substantial inadequacy, as determined by the <br />OCFA, shall be immediately investigated by the OCFA. An example of such <br />situation might be the Contractor's failure to achieve at least a 90% <br />response time performance in any single Code category for a quarterly <br />reporting period. At the discretion of the OCFA, a Corrective Action Plan <br />may be imposed on the Contractor to correct identified and verified <br />performance deficiencies and inadequacies. The OCFA authorized <br />representative shall meet to develop a written Corrective Action Plan (CAP) <br />within fifteen (15) working days of the identification and verification of the <br />service or performance deficiency, or • substantial inadequacy, in <br />accordance with the following CAP requirements: <br />i. CAP Format <br />The CAP shall describe the following:• <br />(1) The service or performance deficiency, or substantial inadequacy <br />shall be identified; and <br />(2) The method by which Contractor is to correct the service or <br />performance deficiency, or substantial inadequacy, shall be <br />outlined. Contractor shall sign the CAP, thereby agreeing to the <br />corrective action set forth in the CAP, with any areas of <br />disagreement noted in writing. A copy of the signed CAP shall be <br />furnished to Contractor at the conclusion of the CAP meeting. <br />ii. Correction Period <br />Correction of the service or performance deficiency, or substantial <br />inadequacy, shall occur within a period of not to exceed sixty (60) <br />days from the development of the CAP, unless the OCFA <br />representative determines that correction cannot be accomplished <br />within the specified time frame.
