• COVID-19 Vaccination Efforts: To ensure the equitable administration of the COVID-19 <br />vaccine reaches Orange County community residents who are the most vulnerable to the <br />virus (e.g., low income; underserved; under and/or uninsured; unemployed; etc.) <br />o December 2020: The Coalition member health centers created a HUB model to <br />vaccinate all their healthcare workers. Through seven HUBs, all the HCWs for the 25 <br />health member health centers were provided first dose vaccines and in the process of <br />receiving second dose. <br />o February 2021 to date: The Coalition of Orange County Community Health Centers <br />(Coalition OC) in partnership with Community Health Centers developed two <br />distinct approaches to administer vaccines to the hardest lit communities and <br />populations. First, we created a centralized approach for Community Health Centers <br />to administer vaccines — through the HUB and SPOKE model. Five Health Centers <br />with the appropriate freezer capacity and infrastructure in place were selected to serve <br />as the primary entity (HUB) to receive vaccine supply from the County Health Care <br />Agency. These five clinics were paired up with the remaining Community Health <br />Center members (SPOKEs) of the Coalition OC in their geographic footprint to work <br />in partnership to administer vaccinations. The HUBS included: 1) Nhan Hoa <br />Comprehensive Health Care Clinic; 2) A1taMed; 3) Families Together of Orange <br />County; 4) Family Health Matters and Serve the People; 5) Southland Integrated <br />Services; and Korean Community Services From February 2020 to date — the HUB <br />and SPOKE Model has been responsible for administering over 80K doses of vaccine <br />to the hardest hit communities. <br />
