City of Santa Ana <br />RFP; Homeiess Navigation Center <br />SCOPE OF SERVICES <br />Overview <br />The Homeless Navigation Center and its supportive service programs will be designed to provide <br />a safe environment and a pathway for individuals, youth and families, experiencing <br />homelessness on the streets of Santa Ana to transition into permanent housing. Clients will be <br />admitted with low -barrier requirements through a referral system and coordination with the Santa <br />Ana Police Department (SAPD), or other outreach teams, with no walk-ins allowed, The shelter <br />program will incorporate transportation pick up and drop off points, or similar model type, that <br />supports the individual needs of participants, The overall project will represent best practices in <br />exceptional trauma -informed care for all shelter participants. The City's obligation will be for <br />funding. The operation of the shelter will be undertaken by the Respondent. <br />Project Requirements <br />Respondents to this RFP should specify programs and services for homeless individuals that, <br />based on evidence, successfully promote the principles of Housing First and contribute to the <br />regional goals of ensuring instances of homelessness are rare, brief, and non -recurring. All <br />services must be easily accessible and evaluated for effectiveness on a regular basis. <br />Effectiveness shall be measured using the Project Outcomes outlined in this RFP. <br />The table below outlines the minimum requirements for the shelter. <br />PROJECTELEMENT <br />Hours of operation . The Navigation Center must be open 24 hours per day, seven <br />days per week, including City holidays <br />Target Population . Housing and Urban Development (HUD) criteria for defining <br />homelessness including Individuals, couples, families with <br />children and transitional aged youth (TAY) populations <br />Client Eligibility + Literally homeless in the City of Santa Ana <br />i Referred by Santa Ana outreach workers and the Santa Ana <br />Police Department <br />Willingness to participate in a housing plan <br />Willingness to follow shelter behavior requirements <br />11 <br />
