SPECIAL REQUIREMENTS (ATTACHMENT 4) <br />This project utilizes California Department of Transportation (Caltrans) funds and shall therefore <br />comply with all state and federal requirements. The below referenced forms included in <br />Attachment 4 (Additional Provisions) of the Appendix must be completed in their entirety <br />and submitted with your proposal: <br />• LAPM Exhibit 10-H1: Sample Cost Proposal <br />• LAPM Exhibit 10-01: Consultant Proposal DBE Commitment <br />• LAPM Exhibit 10-02: Consultant Contract DBE Commitment <br />Please reference Caltrans Local Assistance Procedure Manual, Consultant Selection, Chapter <br />10, for further instructions and guidelines pertaining to the completion of these <br />forms: hitps:Hdot.ca. gov/-/media/dot-media/pro grams/local-assistance/documents/lgpm/ch l 0.pdf <br />COMPLIANCE WITH REQUIREMENTS OF FUNDING AGENCY: <br />This agreement is funded with federal grant funds administered by Caltrans. Proposer shall <br />comply with all requirements as they pertain to the use of these funds. Refer to Attachment 4 for <br />Caltrans required forms, including Exhibit 10-H — Sample Cost Proposal in the Appendix of this <br />RFP. <br />DISADVANTAGED BUSINESS ENTERPRISES (DBE) GOAL: <br />The Agency has established a DBE goal for this Contract. Proposers are encouraged to obtain DBE <br />participation for this contract. Refer to Exhibit 10-I — Notice to Proposers DBE Information <br />included in the Appendix of this RFP. Proposers must submit Exhibits 10-01 & 10-02 — <br />Consultant Proposal & Contract DBE Commitment to demonstrate compliance with Agency's <br />DBE goal. <br />CONSULTANT AUDIT & FINANCIAL REVIEW PROCESS: <br />Consultants, including primes and subconsultants, on a proposed Local Public Agency (LPA) <br />contract that at any time uses state or federal -aid highway funds with a dollar value equal to or <br />greater than $1 Million are subject to a FDR of their ICR by the Independent Office of Audits and <br />Investigations (IOAI). IOAI will review the ICR financial documents to either accept or adjust the <br />ICR prior to contract execution using a risk -based approach. <br />The selected Consultants shall reference and complete the Financial Document Review (FDR) <br />Request Form with the respective financial documents necessary for all prime and sub - <br />consultant rates in the Appendix of this RFP. <br />https:Hi2.dot.ca.2ov/-/media/i2-media/documents/fdr/i2 fdr request form.pdf <br />FDR Requirements for A&E Consultant Indirect Cost Rate: <br />• Requirements for total contract amount equal to or greater than $1,000,000. <br />• Prime and all sub-consultant(s) with estimated contract costs of $500,000 and above must <br />provide the documents marked in the above form based on their applicable category. <br />City of Santa Ana RFP 23-020 <br />City Council age - 0 — 21 7/18/2023 <br />
