ASSUMPTIONS AND/OR EXCLUSIONS USED IN PREPARATION OF <br />THE SCOPE OF WORK AND ASSOCIATED FEE ESTIMATE <br />Survey <br />Assumption: The specifications will be enforced for <br />the surveying requirements. If there is a dispute or <br />error with the Contractor provided Land Surveyor, <br />Coast Surveying is a resource on our team that can <br />assist the City. <br />The Special provisions, 2-9 SURVEYING, 2-9.2 <br />Survey Service states: <br />"The Contractor shall be responsible for <br />directly obtaining the services of a California <br />Licensed Land Surveyor to be in responsible <br />charge of all survey work performed under this <br />contract. " <br />The Special Provision have contract Bid Item No. <br />13 — Survey Monument Adjust to Finished Grade <br />and Bid Item No. 23 — Construction Survey and <br />Staking. <br />Coast Surveying (Coast) is an added value <br />member of our team. Coast fully understands <br />the importance of good service and goes out of <br />their way to provide an extremely high level of <br />very responsive service to their clients including, <br />if needed, resolve a discrepancy, survey bust, <br />monumentation, photogrammetry, hydrographic <br />surveys, GIS services, or create exhibit maps. <br />Coast Survey is available to assist the City and <br />Contractor on this contract as needed. Resumes <br />available upon request. <br />Biologist <br />Assumption: The specifications will be enforced for <br />the environmental survey requirements. <br />The Special Provisions 14-6.03A, Biologists, Yuma <br />Myotis bat and Hoary bat states: <br />"Prior to the start of bridge demolition activities, <br />a qualified bat biologist shall conduct a pre - <br />construction bat survey no more than three days <br />prior to the start of such activities. To avoid direct <br />mortality of individual bats, humane evictions <br />(if bats are present) and exclusions of roosting <br />bats shall be performed under the supervision <br />of a California Department of Fish and Wildlife <br />(CDFW) approved bat biologist prior to bridge <br />demolition activities. Eviction/exclusion activities <br />shall be performed in the fall (September or <br />October) prior to bridge demolition. <br />Exclusion activities may be implemented in <br />one or two phases at the discretion of the <br />qualified bat biologist and in coordination with <br />the City Public Works Director or designee and <br />Contractor." <br />The Special Provisions have contract Bid Item <br />No. 35 — Bird Survey and Bid Item No. 36 — Bat <br />Survey/Exclusion <br />Geode Environmental (Geode) is an added <br />value member of our team. As needed, Geode, an <br />environmental and urban planning firm specializing <br />in environmental law, will provide CEQA/NEPA <br />document delivery, regulatory compliance, <br />permitting, and mitigation. Geode is available to <br />assist the city on this Project. <br />Specialty Testing <br />Assumption: Due to federal funding requirements <br />and to comply with the LAPM, a Source Inspection <br />Quality Management Plan (SIQMP) is required. <br />ZT Consulting Group (ZTC) provides Source <br />Inspection to comply with the LAPM on this Project <br />with Federal Funds. If needed, ZTC specializes <br />in Quality Assurance & Source Inspection (QA, <br />QV, IQA, QMP, SIQMP, QAP and QAM), forensic <br />engineering, and structural health monitoring <br />(SHM).
