Other Information <br />Records and Reports <br />Along with the invoice for services, a report will be submitted to the City, monthly, comprising the following: <br />Daily Log Report of all streets swept and a description of any special services performed. The log will be signed by the <br />Field Supervisor on a daily basis. Each month, a report will be prepared from the daily log giving a brief description of all <br />routine special and emergency activities. The log should also include GPS maps of routes swept, as well as the date, time, <br />and speed with which sweeping occurred each day. <br /> <br /> <br />A phone log of calls from the City’s Public Works Maintenance Services Division and the Police Department, including <br />whether the call contained a request for service and a description of any action taken. <br />Customer services logs and complaints received directly by Athens will be submitted in writing electronically to the City <br />on the day such complaints are received. All complaints are to receive a follow-up response within twenty-four (24) <br />hours. Athens will maintain a log of complaints received and corrective actions implemented which will be submitted to <br />the City each month. <br />Athens will make a reasonable effort to purchase fuel at the City Corporate Yard fueling station at 220 South Daisy Ave, in the <br />City of Santa Ana.
