j <br />2002-2003 EMERGENCY Sk .'TER GRANT PROGRAM <br />APPLICATION FOR SUBRECIPIENT PROPOSALS <br />imha Mental Health Association of Orange County <br />Page i <br />V. <br />GOALS AND OBJECTIVES <br />IMPLEMENTATION STRATEGY <br />In the space below, please list your primary goals and objectives for your program. Provide <br />Projected numbers of Santa Ana residents to be served, and the kinds of services they would <br />receive if your program is funded. Also describe the specific actions and tasks that would be <br />performed on a day-to-day basis to achieve these goals. <br />See Appendix B for examples. <br />The target population for the following goals and objectives are homeless adults suffering from <br />severe and persistent psychiatric disabilities including but not limited to schizophrenia, major <br />depression, schizoaffective disorder and bipolar disorder. <br />GOAL: Increase access to services for the homeless mentally ill that will facilitate their <br />transition into stable housing and appropriate treatment programs. <br />OBJECTIVES : <br />9. Make contact with five homeless Santa Ana residents, (unduplicated), each week and <br />preliminarily screen for mental illness and assess for service needs. <br />Action/Task: <br />(A) Provide ancillary services at a variety of Santa Ana sites each week where homeless <br />individuals gather for services. <br />(B) Provide resources and referrals for the broad spectrum of homeless individuals, focusing on <br />those displaying symptoms of mental illness. <br />(C) Initiate a therapeutic relationship —a process that may take many months when working with <br />the severely and persistently mentally ill. <br />(D) Working with the participant, develop a service plan that takes into consideration mental and <br />physical health needs, shelter, food and clothing, income needs, employment, legal issues, <br />chemical dependency, family and other resources. Screen Program participants for <br />_ _ SSUSSD..!. eligibility within six. months of initial contact. <br />(E) Make appropriate referrals to implement service plan and assist individuals with follow- <br />through on service plan, providing case management and follow up services when needed. <br />(F) Provide EFSP vouchers for motel stays for homeless individuals who meet EFSP <br />and agency criteria. (Approximately 400 total nights of lodging to twenty individuals). <br />Objective number one will assist the target population through their identification and <br />assessment. Referral and other services offered in support of the service plan will help move <br />those encountered through the Program towards greater stability, increasing the chances that <br />they will successfully transition ofrof the streets. <br />2. Develop and maintain working relationships with shelters and other service providers. <br />Action/Task: <br />(A) Participate in Homeless Issues Task Force, Shelter. Providers' Forum, Hunger Coalition, <br />Mental Health Board, and other related committees and task forces. <br />(B) Regularly update lists of shelters and other resources serving the homeless, Make site <br />visits to provider agencies, with Consumers, if appropriate, or independently. <br />(C) Maintain and build upon strong relationships with Ecumenical Service Alliance, The <br />Salvation Army Hospitality House, Southwest Community Center, The Catholic Worker, <br />Orange County Rescue Mission and Shelter for the Homeless. (Please see Attachment 9) <br />(D) Maintain and build upon positive working relationships with the Santa Ana Police <br />Department, Community Development Agency, neighborhood associations and Santa Ana <br />Mental Health and Drug & Alcohol clinics. <br />(E) Develop and expand relationships with other agencies that serve the homeless, and offer <br />in-service training for service providers regarding special needs of the target population. <br />p(thBrr4 <br />
