SOLICITATION # CH16012 <br />5.3 — Acknowledgement of Amendments <br />If the RFP is amended, the Offeror must acknowledge each amendment with a signature on the <br />acknowledgement form provided with each amendment. Failure to return a signed copy of each <br />amendment acknowledgement form with the proposal may result in the proposal being found non- <br />responsive. <br />Carahsoft acknowledges all amendments. Also please find below Carahsoft's Acknowledgement of <br />Amendments form completed. <br />5.4 — Executive Summary <br />Offerors must provide an Executive Summary of its proposal. An Executive Summary should highlight the <br />major features of an Offeror's proposal. Briefly describe the proposal in no more than three (3) pages. The <br />evaluation committee should be able to determine the essence of the proposal by reading the Executive <br />Summary. Any requirements that cannot be met by the Offeror must be included. <br />Please find above Carahsoft's Executive Summary addressing these requirements. <br />5.5 — General Requirements <br />5.5.1 Offeror must agree that if awarded a contract it will provide a Usage Report Administrator responsible <br />for the quarterly sales reporting described the Master Agreement Terms and Conditions, and if applicable <br />Participating Addendums. <br />Carahsoft agrees that if awarded a contract they will provide a Usage Report Administrator responsible for <br />quarterly sales reporting described in the Master Agreement Terms and Conditions, and if applicable <br />Participating Addendums. <br />5.5.2 Offeror must provide a statement that it agrees to cooperate with NASPO ValuePoint and SciQuest <br />(and any authorized agent or successor entity to SciQuest) with uploading an Offeror's ordering <br />instructions, if awarded a contract. <br />Carahsoft agrees to cooperate with NASPO ValuePoint and SciQuest (and any authorized agent or <br />successor entity to SciQuest) with uploading Carahsoft's ordering instructions, if awarded a contract. <br />5.5.3 Offeror must at a minimum complete, provide, and maintain a completed CSA STAR Registry Self - <br />Assessment. Offeror must either submit a completed The Consensus Assessments Initiative Questionnaire <br />(CAIQ), Exhibit 1 to Attachment B, or submit a report documenting compliance with Cloud Controls Matrix <br />(CCM), Exhibit 2 to Attachment B. Offeror must also represent and warrant the accuracy and currency of <br />the information on the completed. Offerors are encouraged to complete and submit both exhibits to <br />Attachment B. <br />For ease of access and evaluation, Carahsoft has provided these Assessment documents under separate <br />cover. All copies have been submitted electronically per the instructions of the RFP.
