<br />- . <br /> <br />'-' <br /> <br />....,j <br /> <br />EXHIBIT A <br />SCOPE OF SERVICES <br /> <br />For the monthly base amount shown in Section 3 of the Agreement CONTRACTOR shall <br />provide the following services: <br />1. PAYMENTS, ANIMAL LICENSE SALES and FEE/FINE COLLECTION <br />(a) CONTRACTOR shall collect all fines and fees due under CITY or State law. <br />(b) CONTRACTOR shall sell CITY animal licenses. <br />(c) CONTRACTOR shall accept cash, personal checks, money order, charge cards, <br />and debit cards (A TM) for all payments and fees and shall provide customers with <br />itemized invoices or receipts. CITY staff shall provide check verification <br />equipment and training to allow CONTRACTOR to accept personal checks. <br />CITY shall reimburse CONTRACTOR for all insufficient funds on the personal <br />checks provided CONTRACTOR uses and operates the equipment according to <br />instructions. <br />2. BOARDING AND CARE <br />(a) CONTRACTOR agrees to provide boarding and care for 5 days (day of impound <br />plus 4 days), in addition to euthanization and disposal if necessary, for all animals <br />collected by CITY personnel and public relinquishments. Such boarding and care <br />shall include 30 cat cages and 42 dog runs. For the purpose of this agreement, <br />"animals" shall mean dogs, cats, and other legal pets. <br />(b) At a minimum, the shelter's hours of operation shall be Monday through Friday <br />noon to 7:00 p.m.; Saturday 10:00 a.m. to 4:00 p.m.; Sunday 10:00 a.m. to 3:00 <br />p.m. The shelter shall be available for animal intake from the public and animal <br />control officers between the hours of 7 am and 11 pm daily. <br />(c) CONTRACTOR shall assign identification numbers to each animal upon intake to <br />the shelter to facilitate adoption and retrieval. Each animal shall be collared with <br />a temporary identification number that shall be supplied by CITY. <br />CONTRACTOR shall provide animal information for public view on each <br />animal's kennel card. Such information shall include animal identification <br />number, arrival date, the date the animal is available for adoption, scheduled <br />euthanasia date, estimated age, sex, breed, and whether the animal has been <br />sterilized. <br />(d) Within one hour of arrival, sick or injured animals shall be examined by a <br />certified, licensed veterinarian. The maximum compensation for medical care to <br />sick and/or injured animals shall be $125.00, including the cost of the initial <br />examination. Contractor shall contact a representative of the City's Animal <br />Control for authorization of all medical treatment. Authorization is not required <br />for immediate or emergency care, however, in such incidents City Animal Control <br />personnel will be notified as soon as practicable. <br />(e) The Contractor shall maintain records on each impounded animal and a daily <br />impound ledger with the following information: ID number; date of impound; <br />description of animal; age; gender; tag information; owner information; and any <br />medical treatment provided, including euthanization date. <br />
