The written release shall be <br />on a form approved by ADMIN ISTRATOR . All subcontractors sha 11 treat all <br />confidential information in the same manner as CONTRACTOR, and in adherence to <br />the terms of this Agreement. <br />8. REPORTS <br />8.1 CONTRACTOR shall prepare and submit to ADMINISTRATOR's designated <br />staff written reports, including, but not limited to: <br />8.1.1 Number of new cases referred each month; <br />8.1. 2 Number of parti ci pants who recei ved servi ces each month, <br />by service type and provider; <br /> <br /> 8.1.3 Number of cases closed each month; <br /> 8.1.4 Number of cases pending at the end of each month; <br /> 8.1.5 Number of cases completing training; and <br /> 8.1. 6 Number of cases obtaining unsubsidized employment. <br />8.2 All reports shall be prepared in a format approved by <br /> <br />ADMINISTRATOR. <br />8.3 ADMINISTRATOR may, in it's sole discretion, add, delete, modify or <br />waive individual reporting requirements as stated in this Paragraph. <br />9. PERFORMANCE REVIEW <br />9.1 ADMINISTRATOR may use a variety of methods to evaluate <br />CONTRACTOR's performance including, but not limited to: <br />9.1.1 Random sampling of program activities including a review <br />of case files; <br />9.1.2 Activity checklists and random observations; <br />9.1.3 Inspection of output items on a periodic basis as deemed <br /> <br />necessary; <br /> <br />9.1.4 Management Information Systems (MIS) reported results: <br /> <br />(WBC0406) <br /> <br />5 of 11 <br /> <br />(April 19, 2006) <br />
