e. A confirmation of the Proposer's willingness and capability to agree to and to follow <br />the terms and processes outlined in this RFP and resulting agreement including all <br />administrative and billing procedures of the City. <br />Any other information the Proposer ieeis is rele~%ant in helping the City gauge the <br />Proposer's business stability. <br />• Identify whether the Proposer had a contract terminated for cause within the last <br />3 years. If so, include an explanation of the circumstances. <br />• Identify whether either a customer or a vendor sued the Proposer in the last 3 <br />years. If so, include an explanation of the circumstances. <br />Identify the percentage of Proposer's business consisting of servicing and <br />supporting NEC telephone equipment <br />g. A description of: <br />• The qualifications, experience and abilities to provide telephone system <br />maintenance services of the Proposer's candidate on-site technician, backup <br />technicians, and any other nn-site engineering and service .staff. <br />The certifications on NEC and CEN"i'IGRAM systems/equipment (as identified in <br />section 4.0) of the Proposer's candidate on-site technician, backup technicians, <br />and any other on-site engineering and service staff. <br />^ Evidence of certifications as part of the proposal. <br />• Any unique qualifications and experience which distinguish Proposer's candidate <br />on-site technician's, backup technicians', and any other on-site engineering and <br />service staff's ability to provide telephone system maintenance services. <br />• The Proposer's candidate on-site technician's and backup technicians' working <br />knowledge of, skills in, and experience with Category 5 cabling and termination in <br />accordance with the Electronic Industries Association/Telecommunications <br />Industry Association (EIA/TIA) standard practices. <br />^ Provide evidence of any related training as part of the proposal. <br />h. Provide a response to the list of services sought by the City by completing the table <br />in the Proposal Response Form, part one-service proposal [see Appendix A1. <br />i. References. Proposer must supply a MAXIMUM of three ~3) references that will <br />attest to the Proposer's ability within the last three years to provide services similar <br />in scope and skills to those outlined in this proposal. For each reference, provide the <br />following information: <br />• The name of the reference organization <br />^ Location <br />• Contact point within the referenced organization <br />^ Name <br />^ Title <br />^ email address <br />^ Telephone number <br />• The approximate period during which the services were provided. <br />• A summary of services provided. <br />6.3 Prise Proposal <br />A Proposal Response Form for the price proposal has been provided in Appendix A, Part 2. <br />This proposal is intended to provide the City with an understanding of the Proposer's <br />charges to provide telephone systems maintenance identified by the City in Section 5.0. <br />Page 75 of 43 <br />
