During the term of this agreement, the Vendor shall provide, at no additional cost, <br />consultation services with regard to such items as: <br />• Upgrades to telephone equipment <br />• Software options <br />• Trunk and Central Office (CO) connectivi~y <br />• Fiber optic installation <br />• Productivity enhancements <br />• VOIP options <br />6.0 Contents of Proposal <br />6.1 Transmittal Letter <br />6.2 Services Proposal with a Description of Qualifications to Perform <br />Required Services <br />A Proposal Response Form for the service proposal has been provided in Appendix A. <br />This proposal is intended to provide the City with an understanding of the Proposer's <br />qualifications to provide telephone systems maintenance (see Section 4.0) and to identify <br />the Proposer's capabilities to provide the specific services sought by the City (see Section <br />5.0). <br />If, in the judgment of the Proposer, the desired objectives of one or more tasks described in <br />Section 5.0 above could be achieved through a different approach, we encourage the <br />Proposer to offer suggestions in its proposal. <br />An important aspect of the services that the City seeks is prior experience in providing <br />telephone system maintenance services. Thus, the quality of prior experience by the <br />proposing firm is very important. To help the City assess the overall qualifications of each <br />Proposer, we ask that the Proposer submit the following materials (See Appendix A for <br />proposal response form): <br />a. Identify prime contact for the proposal (the name, title, address, telephone number, <br />facsimile number, and email). <br />b. The location/s from where the Proposer would provide services to the City. <br />c. A summary description of: <br />• The organization <br />• The ownership <br />• The major service/s offered by the Proposer's firm <br />• The number of years that the Proposer has been in the business of providing <br />telephone system maintenance services, <br />d. A description of: <br />• Overall qualifications of the Proposer and experience and abilit}• to provide <br />telephone system maintenance services. <br />• The firm's certifications/authorizations to provide services on NEC and <br />CENTIGRAM systems/equipment as identified in section 4.0. Also provide <br />evidence of such as part of the proposal. <br />• Any unique qualifications and experience which distinguish Proposer's ability to <br />provide telephone system maintenance services. <br />Page 74 of 43 <br />
