3. Complaintant Responsibility <br />It is the responsibility of the complainant to incl ed at the LWIA hea~ ng wh ch <br />written statement setting forth the facts present <br />support the requested relief. <br />The Chief, WID, shall mail a copy of the request for review to the other party <br />and to the LWIA. Request for Hearing at the State Level <br />If no decision has been issued at the LWIA I the °om e ainanthmay equest ad <br />p <br />an audit disallowance or imposed sanctions, <br />hearing. The hearing officer will then issue a recommended decision to the <br />State's Review Panel for final determinatio <br />2.
