<br /> <br />Orange County 205 25-29 Regional AFH <br /> <br />relevant and affirming support from the City’s network of <br />providers. <br />• Data-Driven Solutions: The City commits to the <br />utilization of data to drive funding decisions and <br />solutions to homelessness. This includes evaluating the <br />efficacy of programs and continual monitoring of the <br />City’s portfolio of interventions to ensure collective <br />efforts are meeting the needs of the community while <br />making gains against agreed upon commu nity goals. <br />The Plan identifies Unsheltered Households, Chronically <br />Homeless Individual Households, Families, Veterans, <br />Transition-Aged Youth, and Seniors as unique <br />populations experiencing homelessness within the City. <br />6. Increase community integration for individuals with <br />disabilities by continuing to assist in the development of <br />housing projects for special needs households by providing <br />technical assistance with tax credit applications, tax-exempt <br />bond financing and other public funds, including, ESG, <br />CDBG, and HOME. <br />On-going, with annual review <br />and adjustments, if <br />adjustments are required and <br />applicable. <br />7. The City shall continue to monitor to ensure the <br />effectiveness of reasonable accommodation standards and <br />procedures and maintain compliance with federal and state <br />housing laws. <br />On-going, with annual review <br />and adjustments, if <br />adjustments are required and <br />applicable. <br />8. In partnership with the Fair Housing Council of OC, provide <br />and maintain multi-lingual informational materials on <br />tenant legal counseling and resources with the goal of <br />eliminating housing discrimination and guaranteeing the <br />rights of all people irrespective of race religion, sex, marital <br />status, ancestry, national origin, color, age, family size or <br />disability to freely choose the housing for which they qualify <br />in the area they desire. <br />9. Continue to participate in City of Anaheim Neighborhood <br />Services Mobile Family Resource Centers as well as <br />agencies and associations who specialize in supporting <br />disabled tenants including those with hearing, vision, <br />cognitive, ambulatory, self - care, and independent living <br />difficulties consistent with data from Anaheim residents <br />reporting a disability. <br />Continue to provide estimated <br />annual allocation of $100,000, <br />based on program funding <br />availability. <br />Annually evaluate program <br />effectiveness and adjust Scope <br />of Services as appropriate and <br />based on available funding. <br />Target an increase in the <br />number of households served <br />per funding dollar. <br /> <br />EXHIBIT 6
