<br /> Administrative Plan 7/1/2025 Page 18-12 <br /> The FYI voucher holder is a person who is regularly and actively participating in a drug <br />addiction or alcohol treatment and rehabilitation program. <br />SAHA Policy <br />SAHA will define regular and active participation in a manner that provides extensions <br />of FYI voucher holder assistance to the broadest population possible consistent with the <br />statutory requirements. <br />The FYI voucher holder will be required to self-certify that they meet this exception on a <br />SAHA-provided form. This certification is the only documentation that the FYI voucher <br />holder must submit. <br /> The FYI voucher holder is a person who is incapable of complying with the requirement to <br />participate in a FSS program as described above or engage in education, workforce <br />development, or employment activities as described below, as applicable, due to a <br />documented medical condition. <br />SAHA Policy <br />SAHA will apply this requirement in a manner that provides extensions of FYI voucher <br />holder assistance to the broadest population possible consistent with statutory <br />requirements. <br />The FYI voucher holder will be required to self-certify that they meet this exception on a <br />SAHA-provided form. This certification is the only documentation that the FYI voucher <br />holder must submit. <br />An FYI voucher holder that meets one of the above exceptions must still be offered an <br />opportunity to enroll in the PHA’s FSS program (if it is available to them) and receive any <br />supportive services available to FYI voucher holders. An FYI voucher holder may choose to <br />participate in an FSS program or engage in education, workforce development, or employment <br />activities, even if they meet one of the above statutory exceptions. <br />EXHIBIT 1
